胫骨平台骨折关节镜下微创治疗的疗效分析 被引量:18

摘要 目的分析胫骨平台骨折关节镜下微创治疗的临床疗效。方法总结133例胫骨平台骨折病例,根据治疗方法分为常规手术组71例和关节镜组62例,比较两组患者切口长度、术中出血量、手术时间、解剖复位率、早期晚期并发症以及骨折愈合时间;通过Sanders膝关节功能评分法,比较临床治疗效果。结果两组手术时间、解剖复位率、早期晚期并发症比较无显著区别;与常规手术组比较,关节镜组手术切口长度短、术中出血量少、愈合时间短、术后疗效优良率高(P<0.05或P<0.01),其中SchatzkerⅣ型患者Sanders膝关节功能评分优于常规手术组(χ2=5.560,P<0.05)。结论关节镜下微创治疗胫骨平台骨折具有损伤小、骨折愈合快、疗效佳的特点,是一种有效的治疗方法。 Objective To analyze the clinical curative effects of arthroscopic minimally-invasive treatment on fracture of tibial plateau.Methods The data of 133 cases of tibial plateau fractures were summarized.According to the methods of treatment,they were divided into two groups:conventional operation group(71 cases)and arthroscopic treatment group(62 cases).Comparison was made in the length of incision,blood loss during the operation,operative time,anatomical reduction rate,early and late complications and time of fracture healing between the two groups.The clinical curative effects of the two groups were also compared through Sanders knee joint scoring.Results There was no significant difference in the operative time,anatomical reduction rate and early and late complications between the two groups.Compared with the conventional operation group,the arthroscopic treatment group had a shorter incision length,less blood loss during the operation,shorter time of fracture healing and higher excellent rate of postoperative curative effects(P〈0.05 or P〈0.01).The Sanders knee joint function scores of patients with Schatzker Ⅳ were significantly higher than those of patients in the conventional operation group(χ2=5.560,P〈0.05).Conclusion Arthroscopic minimally-invasive treatment on patients with fracture of tibial plateau has characteristics of less injury,quick fracture healing and good curative effects,which is an effective method of treatment.
