摘要
目的:探讨二尖瓣环结构异常与二尖瓣反流程度的关系。方法:运用飞利浦IE33彩超仪采集41例二尖瓣反流患者的实时三维超声心动图二尖瓣环图像,根据二尖瓣反流程度分为轻度反流组(18例)与重度反流组(23例)两组;选取21例二尖瓣无反流的正常人作为对照组。结果:正常对照组与轻度反流组的二尖瓣环三维构型均呈马鞍形态,且在整个心动周期中保持马鞍形,而重度反流组的二尖瓣环在整个心动周期中,呈扁平圆形。结论:病理性二尖瓣反流除了二尖瓣叶病变以外,二尖瓣环的三维立体形态改变可能导致不同程度的二尖瓣反流。
Purpose: To explore the relationship between structural abnormalities of the mitral annulus and the severity of mitral regurgitation(MR).Methods: Forty-one patients with MR were divided into two groups: group with mild MR(18 cases) and group with severe MR(23 cases) according to the degree of the regurgitation.Twenty-one healthy subjects without significant MR were considered as control.Dynamic images were collected by Philips IE33 ultrasound instrument with RT-3DE.Results: A saddle shape of mitral annulus in three-dimensional geometry was observed in the control and mild MR groups,and the shape kept its saddle like shape throughout the cardiac cycle in both control and mild MR groups.Oblate pattern of mitral annulus was existed throughout the cardiac cycle in sever MR group.Conclusion: Changes in three-dimensional shape of mitral annular could lead to different degrees of MR in addition to pathological changes of mitral valve.
