呼吸衰竭患儿下气道感染G^-杆菌的分布与变迁 被引量:3

摘要 目的分析儿科重症监护病房(PICU)呼吸衰竭患儿下气道感染G-杆菌的分布、变迁及耐药情况,为指导临床合理应用抗生素提供依据。方法回顾性分析2007年1月~2009年12月我院PICU病房因呼吸衰竭而行机械通气的336例患儿的痰培养结果,采用法国生物梅里埃VITEK-32微生物鉴定系统鉴定细菌菌种,以纸片扩散法进行药敏试验。结果共分离出G-杆菌204株,检出主要的5种病原菌分别为肺炎克雷伯菌69株(33.83%)、鲍曼不动杆菌55株(26.96%)、铜绿假单胞菌50株(24.51%)、嗜麦芽窄食单胞菌15株(4.46%)和肠杆菌15株(4.46%)。鲍曼不动杆菌占检出G-杆菌的构成比在逐年增加(P<0.01);肺炎克雷伯菌属中产超广谱β-内酰胺酶(ESBLs)的总体检出率为39.13%,产酶率有逐年增高趋势(P<0.05)。药敏结果显示,G-杆菌对常用抗生素均存在不同程度的耐药,其中鲍曼不动杆菌和铜绿假单胞菌对亚胺培南的耐药率在逐年增加,嗜麦芽窄食单胞菌对碳青酶烯类天然耐药。结论 PICU病房以G-杆菌感染为主,不同病原菌对常用抗生素的敏感性和耐药性差异较大,及时掌握本病房病原菌的分布和变迁特点,依据可靠的病原学检查,可以为临床合理选择抗生素提供有益依据。
