头部亚低温对脑出血患者炎性损伤干预作用的研究 被引量:5

摘要 目的观察脑出血患者白细胞介素-1β(IL-1β)、细胞间粘附分子-1(ICAM-1)和肿瘤坏死因子-α(TNF-α)含量的变化及头部亚低温对其的影响。方法 86例脑出血患者根据是否加用头部亚低温治疗分为头部亚低温组(45例)和对照组(41例),采用酶联免疫吸附法(ELISA)测定两组患者治疗前及治疗后7d血清1L-1β、ICAM-1、TNF-α含量,并与正常组(30例)进行比较。结果头部亚低温组和对照组治疗前1L-1β、ICAM-1、TNF-α明显升高,与正常组比较差异均具有显著性(P<0.01);治疗前头部低温组和对照组间1L-1β、ICAM-1、TNF-α差异均无显著性(P>0.05);治疗后7d,头部亚低温组1L-1β、ICAM-1、TNF-α较治疗前明显降低(P<0.05),对照组降低不明显(P>0.05),头部亚低温组与对照组比较差异均具有显著性(P<0.01)。结论脑出血患者早期血清1L-1β、ICAM-1、TNF-α含量明显升高,头部亚低温组可有效降低其含量。 Objective To study changes in interleukin-1β(1L-1β),intercellular adhesion molecule-1(ICAM-1)and tumor necrosis factor-α(TNF-α)levels in intracerebral hemorrhage(ICH)and observe potential effect of head mild hypothermia.Methods Eighty-six cases of intracerebral hemorrhage(ICH)were divided into head mild hypothermia treatment group(n=45)and control group(n=41)according to treatment with or without head mild hypothermia.The serum 1L-1β,ICAM-1 and TNF-α level in the twe groups were determined on the 7 th days with enzyme linked immunoadsorbent assay(ELISA),and compared to those of the healthy volunteers(n=30).Results The serum 1L-1β,ICAM-1 and TNF-α level in both head mild hypothermia treatment group and control group were higher before treatment compared with the healthy controls(all P〈0.01),and there was no significant difference in serum 1L-1β,ICAM-1 and TNF-α level before treatment between the head mild hypothermia and control group(both P〉0.05).Serum 1L-1β,ICAM-1 and TNF-α level in the head mild hypothermia treatment group on the 7 th day after the head mild hypothermia were significantly lower than befor the head mild hypothermia treatment(P〈0.05),while the changes of control group was not significant(P〉0.05),and significant ifference in serum 1L-1β,ICAM-1 and TNF-α was found between head mild hypothermia treatment group and control group on the 7 th day(P〈0.01).Conclusion The levels of serum 1L-1β,ICAM-1 and TNF-α were increased significantly during early stae of ICH.Head mild hypothermia can effectively reduce the serum 1L-1β,ICAM-1 and TNF-α levels of early ICH patients.
