摘要
目的:观察左旋精氨酸(L-Arg)对正常人和肥胖者胰岛早期分泌相的兴奋作用,为各种临床背景下胰岛细胞早期分泌功能变化提供依据。方法:选择92例健康者,其中肥胖者38例(BMI>25),正常体重者54例,L-Arg5g快速静脉注射,观察注射前及注射后2分、4分、6分胰岛素(INS)、C-肽(C-P)和胰升血糖素(GC)水平。结果:发现空腹INS、C-P和GC肥胖组明显高于正常体重组(P<0.05或P<0.01)。L-Arg兴奋后INS、C-P和GC分泌均在2分达峰值,肥胖组INS、C-P和GC峰值浓度虽高于正常体重组(P<0.05和P<0.01),但前者峰值较空腹值增加的幅度却低于后者。4分和6分时C-P/INS比值也是前者低于后者(P<0.05)。从单相关分析发现,BMI与空腹INS、C-P和GC成正相关(P<0.05或0.01),与2分时INS、C-P和GC值无明显相关性。逐步回归分析发现只有空腹C-P和GC进入回归方程。结论:L-Arg对人体胰岛α和β细胞有较强的兴奋作用,BMI是影响胰岛分泌激素的主要因素。
Objective:To explicate the early islet function of obesity and healthy persons stimulated by L-Arginine(L-Arg). Meth-ods: 38 obesity(OB group)and 54 healthy persons(control group)were observed. The test of L-Arg was used. Results; As for fasting insulin,C-peptide and glucagon.the values of them were higher in OB group than in control group. When the islet function was stimulated by L-Arg,the values of insulin,C-peptide and glucagon at two minutes in OB group were higher than the control group (P<0.05 or P<0.01). Correlation analysis showed that BMI presented positive correlation with INS,C-P and glucagon,multiple regression analysis indicated that fasting C-P and glucagon candidated in equation(P< 0.05).Conclusion;The results suggested that the response to L-Arg in obesity person was increased, and it reflected the high secretion of αcell and β cell in islet and disturbance of islet function. BMI was the main factor influencing the islet function.
