磁共振在肩关节撞击综合征诊断中的应用价值 被引量:14

摘要 目的评价1.5T MRI对肩关节撞击综合征的诊断价值,并分析、总结其MRI征象及损伤机制。方法回顾性分析7例经关节镜证实为肩关节撞击综合征患者的MRI及临床资料。结果①直接征象:7例患者均有冈上肌腱形态及信号的改变,完全撕裂者1例,部分撕裂者3例,无法确定有无撕裂者3例;②间接征象:肩峰下滑囊增厚者2例,肩峰下滑囊积液者3例;③继发征象:关节盂唇撕裂者1例,冈上肌或三角肌萎缩者0例;④病因学征象:7例患者中肩峰为钩状者4例,扁平状者1例,弯曲状者2例;肩峰下间隙<6mm者3例。结论 1.5T MRI能有效显示肩关节撞击综合征的产生原因和损伤程度,在肩关节撞击综合征的诊断中具有较高的价值。 Objective To evaluate the diagnostic value of 1.5T MRI in shoulder impingement syndrome(SIS),then analyze and summary the MRI features and injuring mechanism of SIS.Methods 7 cases of SIS confirmed by arthroscopy underwent MRI,and the MRI findings and clinical symptoms were reviewed retrospectively.Results ①Direct signs:Supraspinatus tendon illustrated signal and patterns changes in all 7 cases,while supraspinatus tendon full-thickness tears were detected in 1 patient.Partial thickness tears were found in 3 patients.3 cases could not be determined whether their tendon tear.②Indirect signs:Subacromial bursa thickened in 2 cases,while subacromial deltoid sac fluid in 3 cases.③Secondary signs:Included 1 case of joint labral avulsion and 0 case of the supraspinatus and deltoid atrophy.④Etiological signs:Acromino were classified as flat,smoothly curved and hooked.Conclusion 1.5T MRI can effectively show the causes of shoulder impingement Syndrome and the degree of injury,has a higher value in the diagnosis of SIS.
