摘要
目的研究瑞芬太尼不同效应室靶浓度对患者异丙酚镇静效应的影响。方法将75例择期全麻手术患者随机分为A、B、C、D、E共5组,脑电双频指数(BIS)降为50时,记录异丙酚效应室靶浓度、异丙酚总用量以及所需时间,以及患者意识消失时BIS值和异丙酚效应室靶浓度。结果 5组患者麻醉诱导开始各基础值无显著差异。BIS值降为50时C、D、E组分别与A组、B组相比,异丙酚效应室靶浓度、异丙酚总用量以及所需时间均明显减少;患者意识消失时C、D、E组分别与A组、B组相比,异丙酚效应室靶浓度明显降低,BIS值明显增高。结论瑞芬太尼与异丙酚复合用于全身麻醉时适宜的效应室靶浓度为4 ng/mL。
Objective To study the effects of different target effectsite concentrations of remifentanil on propofol sedation.Methods A total of 75 patients who received elective surgery under general anesthesia were randomly divided into groups A,B,C,D and E.When bispectral index(BIS) reduced to 50,the propofol effect,target effect-site concentrations,total propofol dosage,time required,and BIS values and target concentrations when patients lost consciousness of propofol effect were recorded.Results The basic values at the onset of anesthetic induction showed no significant differences between the 5 groups.As the BIS values dropped to 50,the effect of propofol target concentration,propofol dosage and time required in groups C,D,E were significantly less than those in groups A and B.When patients lost consciousness,compared with the groups A and B,the effect of propofol target concentration was significantly lower,and the BIS value was significantly higher in groups C,D and E.Conclusion The optimal target concentration is 4 ng/mL when combined remifentanil-propofol is administered for general anesthesia.
