摘要
目的以彩超动态检测肾血管性高血压犬颈动脉结构和血流动力学的改变。方法首先以双肾双夹法(2K2C)建立肾血管性高血压犬模型,然后应用彩超动态检测缩窄犬(N=5)及假手术犬(N=5)颈动脉内径、内膜中层厚度(IMT)、血流收缩期峰值流速(PSV)、阻力指数(RI)、斑块发生率和斑块积分(Crousescore)的变化。结果术前缩窄犬的平均收缩压(SBP)为(138.80±9.20)mmHg,术后3月达到峰值171.20±3.56mmHg,此后保持相对平稳直到术后12月,保持在(168.60±8.88)mmHg水平,和假手术组及基线值相比均达统计学差异(P〈0.01orP〈0.05)。术后第7天,缩窄犬及假手术犬的颈动脉IMT分别为(0.27±0.03)mm和(0.26±0.03)mm,两组均未检出斑块;至术后12月,两组IMT分别增厚为(0.49±0.01)mm和(0.38-4-0.03)mm,5缩窄犬检出17个斑块,假手术犬有2只检出4个小斑块。组内比较:缩窄犬术后4月及术后12月颈总动脉的内径、IMT及RI均明显高于术后第7天的值(P〈O.01);假手术犬在术前、术后各指标的差异均无统计学意义;组问比较:缩窄犬的IMT在术后4月及术后12月(P〈0.01),RI(P〈0.05)、斑块检出率(P〈0.05)及斑块积分(P〈0.01)在术后12月均明显高于假手术犬;余指标无统计学意义。结论肾血管性高血压犬的颈动脉存在粥样硬化性结构及血流动力学改变;彩超检测可以动态观察其颈动脉粥样硬化的发生和发展。
Objective To dynamically detect the structure and hemodynamic changes of carotid arteries in renovascular hypertensive canine by color ultrasound. Methods The renovascular hypertensive canine model was established by 2 kidney 2 clip (2K2C) method, and the color ultrasound was used to dynamically detect the changes of the inner diameter, intima media thickness (IMT), peak systolic velocity (PSV), resistent index (RI), plaque incidence and Crouse score of carotid arteries in constricted and sham-operated beagles (n=5, respectively). Results The mean Systolic blood pressure (SBP) in constricted beagles was (138.80 ± 9.20) mmHg before operation, and reached a peak valueof (171.20 ± 3.56) mmHg three months after operation. Afterward, the SBPs remained stable for up to 1 year, the average SBP was still (168.60 ± 8.88) mmHg, which was signifieantly higher than the baseline and sham control values (P〈0.01 or P〈0.05, respectively). On the 7th postoperative day, the carotid IMT of constricted and sham-operated beagles were (0.27 ± 0.03) and (0.26 ± 0.03) mm respectively, and no carotid plaques were detected in 2 groups; twelve months after operation, the IMT in 2 groups IMT inerassated to (0.49 ± 0.01) and (0.38 ± 0.03) mm respectively, and 17 carotid plaques were identified in 5 test beagles, while 4 plaques in 2 of 5 sham animals. The inner diameter, IMT and RI in the postoperative 4th and 12th month were respectively higher than those in the postoperative 7th day within constricted beagles (Pall 〈0.01), but no significant difference was found in the parameters between preoperation and postoperation within sham-operatedanimals. The IMT in the postoperative 4th and 12th month, and the RI, plaque detection rate and Crouse score in the postoperative 12th month in test beagles were respectively higher than those in sham ones, and there was no significant difference in other parameters between 2 groups. Conclusions The atherosclerotic structure and hemodynamic changes were presented in the carotid arteries of renovascular hypertensive beagles; and the color ultrasound could be used to dynamically identify the occurrence and development of carotid atherosclerosis.
