摘要
目的 探讨应用接触式激光显微手术治疗颅底脑膜瘤的治疗效果。方法 应用接触式激光显微手术治疗颅底脑膜瘤 2 0例 ,与传统显微手术方法治疗颅底脑膜瘤 2 6例进行临床效果分析。结果 激光组 17例全切 ,2例近全切 ,1例大部分切除 ,术中平均输血 810ml;而对照组 15例全切 ,7例近全切 ,4例大部分切除 ,平均输血 10 70ml,P <0 0 5。前者无 1例病情加重或死亡 ,随访半年至 2年 ,1例岩 -斜坡脑膜瘤术后 1年复发 ;后者术后症状改善 12例 ,加重 3例 ,死亡 2例 ,随访期同上 ,有3例 1年后肿瘤复发。结论 接触式激光适用于供血丰富的肿瘤切除 ,对颅底脑膜瘤等脑深部肿瘤手术切除 ,能减少术中出血和对周围组织的损害 ,避免术后并发症发生 ,降低死亡率 ,提高颅底脑膜瘤的全切除率。
Objective To investigate the advantages of using contact laser in treatment meningoma in base of skull to improve the surgical effect. Methods Twenty patients with meningoma in base of skull were treated microsurgically using contact laser. Twenty-six patients with meningoma in base of skull were treated with generally microsurgical technique as control. Results Total resection was achieved in 17 cases, subtotal in 2 cases and partial in 1 case. The average amount of blood transfusion for each patients was 810 ml. There was no surgical mortality. All patients were followed up for 6 months to 2 years. There were no patients getting worse during the following up period in the laser treatment group.In the control group,total resection was achieved in 15 cases,subtotal in 7 cases and partial in 4 cases. The average amount of blood transfusion for each patient was 1 070 ml( P <0 05).Two patients died and 3 patients were worse after surgery. Three patients recurred after one year. Conclusion The contact laser can precisely dissect and vaporize the tumors, without peritumoral normal brain tissue injury and with very mild postoperative brain edema,and has excellent hemostatic effect. The side effect is avoided and the surgical mortality was low. [WT5”HZ]
