摘要
目的:了解门诊处方存在的问题,规范处方书写,提高处方质量,以促进合理用药,减少药品不良反应。方法:对上海市中医医院2011年7—10月的门诊处60 835张,根据卫生部2007年颁布的《处方管理办法》、2010年颁布的《医院处方点评管理规范》、《中华人民共和国药典.临床用药须知》(2010年版)、《新编药物学》(第16版)和药品说明书中的规定进行统计分析与评价。结果:门诊处方存在书写不规范、不合理用药问题,合格处方60 615张,不合格处方220张,处方合格率为99.6%。结论:医师和药师应加强专业知识的学习,提高医师书写处方和合理用药的能力,提高药师审核处方的能力,从而提高门诊处方的质量,确保合理用药。
OBJECTIVE：To probe into the problems of outpatient prescriptions,standardize prescription writing and improve prescription quality so as to promote rational drug use and reduce incidence of adverse drug reactions.METHODS： A total of 60 835 outpatient prescriptions in Shanghai Hospital of Traditional Chinese Medicine from July to October in 2011 were subjected to statistic analysis and evaluation in accordance with ＂Prescription Management Measure＂ issued by Ministry of Health in 2007,＂Management Standard on Hospital Prescription Comment＂ issued in 2010,＂Pharmacopoeia of the People＇s Republic of China-Clinical Dispensing Information＂（2010 Edition）,the ＂New Edition of Pharmacology＂（16th Edition） and drug package inserts etc.RESULTS： Nonstandard writing of prescriptions and irrational drug use were noted in outpatient prescriptions.Of the 60 835 outpatient prescriptions reviewed,60 615（99.6%） were qualified and 200 were unqualified.CONCLUSION： Both physicians and pharmacists should strengthen learning on professional knowledge in order to improve the ability in prescription writing for physicians and rational medication and the ability in checking prescription for pharmacists so as to improve the quality of outpatient prescriptions and ensure rational drug use.
