摘要
目的探讨不同剂量间苯三酚在产程活跃期的临床应用效果。方法选择2009年1月～2011年2月本院入院的无妊娠并发症及合并症的初产妇200例,随机分为3组,一组给予间苯三酚40 mg干预(69例),二组给予间苯三酚80 mg干预(66例),三组给予间苯三酚120 mg干预(65例)。观察不同剂量间苯三酚对产妇、产程及胎儿的影响。结果①妊娠结局:所有产妇均经阴道分娩,均无产后大出血情况发生。3组产妇均有2例出现羊水Ⅱ度污染;二组有2例出现羊水Ⅰ度污染,出现1例胎儿宫内窘迫,经对症处理后胎儿情况恢复正常并顺利分娩;三组出现羊水Ⅰ度污染1例,Ⅲ度污染1例。②产程活跃期:应用间苯三酚120 mg产妇产程活跃期为(76.3±16.7)min,与一、二组比较差异有高度统计学意义(P<0.01);与一组比较,二、三组产程活跃期明显缩短,差异有高度统计学意义(P<0.01)。③宫口开大:用药40 min后,三组产妇进入产程活跃期,宫口开大情况差异有统计学意义。④产后出血及新生儿情况:三组产妇产后出血、胎心率、新生儿体重及Apgar评分差异无统计学意义(P>0.05)。结论间苯三酚在分娩过程中可明显缩短产程,用药安全,大大降低剖宫产率,值得在产科中推广。
Objective To investigate the clinical application of phloroglucinol on different dosage in active phase labor.Methods 200 cases primiparas with no pregnancy complications were chose from January 2009 to February 2011,and all patients were randomly divided into 3 groups,the first group was given phloroglucinol 40 mg intervene with 69 cases,the second group was given phloroglucinol 80 mg intervene with 66 cases,the third group was given phloroglucinol 120 mg intervene with 65 cases,the influence of primipara,birth process and embryo of three dosages were observed.Results ①Pregnancy outcome： All lying-in woman had no postpartum hemorrhage,there were 2 primiparas respectively of three groups meconium-stained amniotic fluid Ⅱ degree,in the second group,there were 2 primiparas meconium-stained amniotic fluid Ⅰ degree,in the third group,there was 1 primipara meconium-stained amniotic fluid Ⅰ degree,1 case Ⅲ degree.②Active phase labor： The active phase labor of the third group was（76.3±16.7） min,compared with the first and second group,there were significant differences（P〈0.01）,compared with the first group,the third group had significant differences（P〈0.01）.③Cervical open： After 40 min,primiparas of three groups were in active phase labor,the situation of cervical open had significant differences.④Post partum situation： there were no significant differences among postpartum hemorrhage,fetal heart rate,fetal weights and Apgar grade of three groups.Conclusion It can shorten the birth process using phloroglucinol,the drug is safe,and can reduce the cesarean section rate,and is worth on maternity clinical.
