摘要
目的探讨血清同型半胱氨酸(HCY)、踝臂指数(ABI)和肱踝脉搏波传导速度(baPWV)与高血压前期的关系。方法入选214例体检人群,根据血压水平分为理想血压组(60例)、高血压前期1组(78例)、高血压前期2组(76例),采用荧光偏振免疫分析法(FPIA)测定HCY,VP1000动脉硬化检测仪检测ABI、baP-WV。结果理想血压组、高血压前期1组、高血压前期2组HCY、baPWV分别为[(11.34±4.65)μmol/L]、[(10.52±1.07)m/s],[(13.27±3.72)μmol/L]、[(13.18±2.14)m/s],[(14.98±5.13)μmol/L]、[(15.63±2.87)m/s],三组间差异均有统计学意义(P<0.05)。高血压前期1组、2组ABI分别为(1.05±0.12)、(0.97±0.16),两组差异无统计学意义;高血压前期1组、2组ABI与理想血压组(1.18±0.25)比较,差异有统计学意义(P<0.05)。Logistic回归分析表明,体重指数、HCY、baPWV为理想血压进展为高血压前期的独立危险因素。结论 HCY、ABI、baPWV与高血压前期有关,三者联合可用来更好的评估高血压前期及其危险程度。
Objective To discuss the relationship between prehypertension and serum homocysteine(HCY),ankle-brachial index(ABI) and brachial-ankle pulse wave velocity(baPWV).Methods All 214 cases with physical examination were selected and divided into ideal blood pressure group(n=60),prehypertension group 1(n=78) and prehypertension 2(n=76) according to their blood pressure level.The level of HCY was detected by using fluorescence polarization immunoassay(FPIA),and changes of ABI and baPWV were detected by using VP1000 arteriosclerosis detector.Results In ideal blood pressure group,prehypertension group 1 and prehypertension 2,the level of HCY was,respectively,[(11.34±4.65) μmol/L],[(13.27±3.72) μmol/L] and [(14.98±5.13) μmol/L],and baPWV was,respectively,[(10.52±1.07) m/s],[(13.18±2.14) m/s] and [(15.63±2.87) m/s].The difference was statistically significant among three groups(P〈0.05).In prehypertension group 1 and prehypertension 2,ABI was,respectively,(1.05±0.12) and(0.97±0.16) and the difference was not statistically significant.The comparison of ABI between prehypertension group 1 or prehypertension 2 and ideal blood pressure group(1.18±0.25) showed that the difference was statistically significant(P〈0.05).The results of Logistic regression analysis indicated that body mass index,HCY and baPWV were independent risk factors in the development from ideal blood pressure to prehypertension.Conclusion HCY,ABI and baPWV are related to prehypertension,and they can be used jointly for evaluating prehypertension and risk factors better.
