摘要
目的:评价健康和疾病定量测试法大众版(RTHD-P)在精神障碍流行病学调查中的信度和效度,以及用于健康与疾病状态筛查和帮助建立居民健康档案的适用性。方法:应用分层整群抽样抽取宁波市6个社区和6个自然村的15岁以上常住人口650名,分别代表城市和农村。先用RTHD-P进行筛检,同时分别用RTHD-知情人评定表(RTHD-PI)和RTHD—临床评定者量表(RTHD-PC)对同一受检者评定,进行平行信度分析。然后用诊断量表RTHD—科研版(RTHD-R)复核(效度检验),以验证RTHD-P作为筛检工具的科学性和效度分析。在评估后的7～14天内,由另一名研究者对随机抽取的200例患者进行第二次RTHD-P评定,以评价RTHD-P的重测信度。结果:RTHD-P各个分量表的Cronbach'sα系数在0.7231～0.8751之间,但在性病、吸毒、精神病等条目中需要知情人评定和临床评定员的进一步访谈,方能确定相关信息。经诊断量表RTHD-R的效度检验显示,在心理健康、躯体健康和社会功能因子kappa=0.97-1.00(P<0.01)。结论:RTHD-P 3种版本具有很好的信度。通过正确使用RTHD-P的3种版本,经RTHD-R验证,可以提高在躯体健康、心理健康和社会功能方面的评定效度,值得在流行病学调查及疾病筛查中推广应用,并适于帮助建立居民健康档案。
Objective To test the reliability,validity,practicality and acceptability of RTHD-P.Methods A stratified cluster sampling technique was conducted to select a sample of 650 permanent inhabitants,aged more than 15 years old,from 6 rural villages and 6 urban districts of Ningbo.RTHD-P,RTHD-PI and RTHD-PC was carried out on the same subject to evaluate the parallel reliability.RTHD-R was used as the gold standard to test the diagnostic validity of RTHD-P,and to test the scientificity and practicality of RTHD-P as a screening instrument.After the first assessment,200 patients were randomly selected for the second examination of RTHD-P by other researchers in 7-14 days,to evaluate test-retest reliability of RTHD-P.Results Cronbach＇s α coefficients of subscales of RTHD-P were between 0.7231 and 0.8751.However,the items of sexually transmitted diseases,drug addiction,mental illness need insiders＇ assessment and further clinical assessment to determine the relevant information.The validity test showed that the kappa was 0.97-1.00（P0〈.01） in terms of mental health,physical health and social function.Conclusion RTHD-P has satisfied reliability in three versions.The validity of the assessment of physical health,mental health and social function can be improved through the proper use of the three versions of RTHD-P by RTHD-R certification.RTHD-P is worthwhile to be widely used in epidemic surveys and disease screening and is suitable for helping keep the health files of residents.
