摘要
目的:通过检测细胞粘附分子(CD44v6)及增殖细胞核抗原(PCNA)在大肠癌不同中医证型中的表达,探讨CD44v6、PCNA与虚实证型的关系。方法:采用前瞻性组间对照的研究方法,筛选符合标准的大肠癌患者51例和大肠息肉患者17例,术前认真辨证,术后标本免疫组化检测各种证型组织中CD44v6及PCNA的表达。结果:息肉组织中CD44v6无表达或呈弱表达,大肠癌组织中CD44v6表达明显增强。大肠癌虚证与实证中有淋巴结转移或全身多处转移的患者,CD44v6的表达有显著差异性(P<0.05),实证占85.7%,虚证占46.2%,而PCNA的表达无差异性(P>0.05)。结论:CD44v6、PCNA在大肠癌中的表达具有较高的特异性。CD44v6作为细胞黏附蛋白分子,同肠癌的发生与发展可能有关,对大肠癌预后的判断具有重要的参考价值。PCNA在大肠癌进展期表达具有特征性意义。大肠癌实证中有淋巴结转移或全身多处转移的患者CD44v6的表达明显高于虚证中的表达,提示CD44v6及转移倾向与邪气的盛衰相关。
dObjective To explore the significance of CD44V6 and PCNA, and to reveal the essence as well as the gene relationship by detecting the expression of CD44V6 and PCNA in different patterns of colonrec- tal cancer on TCM syndrome differentiation. Method A prospective controlled study method was introduced. Fifty one colonrectal cancer patients and 17 patients with colonrectal polypus were divided into two groups rela- tively.The dialectical information was recorded preoperatively and all the colonrectal cancer patients had been pa- thology examinated postoperatively, CD44V6 and PCNA were immunohistoehemically detected. Result CD44v6 was highly expressed in colorectal carcinoma, while in colonreetal polypus, the expression was not obvi- ous, with significant differences(P〈0.05).The PCNA was highly expressed in both eolorectal carcinoma and poly- pus, the distribution was heterogeneous. The expression of CD44v6 in colonrectal cancer with positive lymph node and multiple metastasis was significantly different between excessive and deficient patterns(P〈0.05), the expression rate in excessive patterns was 85.7%,while in deficient patterns 46.2%, and the expression of PC-NA had no distinguish between thetwo groups.Conclusion The expression of CD44v6 and PCNA in colorectal carcinoma has a high specificity. CD44v6 may have correlation with the occurrenceand development of colorectal cancer, well as the prognosis. The expression of PCNA in colorectal carcinomaprogression has a characteristic.The expression of CD44v6 in colonrectal cancer excessive patterns with positivelymph nodes and multiple metastasis is significantly higher than there in the deficiency patterns, which reflectsdeficiency-excess theory of TCM. CD44v6 is also related to transfer orientation and up and down of pathogenicqi in TCM syndrome diagnosis.
