摘要
目的探讨脑梗死患者载脂蛋白E(Apo E)基因多态性分布及其与C反应蛋白(CRP)、美国国立卫生院卒中量表(NIHSS)的关系。方法收集脑梗死108例(其中急性脑梗死70例,非急性脑梗死38例)和健康对照者76例,利用多聚酶链式反应-限制性片段长度多态性(PCR-RFLP)技术,测定其Apo E基因型;采用生化全自动仪分别对总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)和CRP水平进行测定;对所有脑梗死患者入院时、入院后1周和2周进行NIHSS评分。结果脑梗死患者Apo Eε2、Apo Eε4的频率分别为6.9%和9.7%,而对照组分别为19.1%和3.9%,差异均有统计学意义(P<0.05);脑梗死组TC、TG、LDL-C明显高于对照组,而HDL-C明显低于对照组(P<0.05);急性脑梗死组Apo Eε4携带者的CRP明显高于非Apo Eε4携带者(P<0.05);脑梗死患者Apo Eε4携带者的NIHSS评分高于非Apo Eε4携带者的NIHSS评分(P<0.05);急性脑梗死患者Apo Eε2携带者的NIHSS评分低于非Apo Eε2携带者的NIHSS评分,而急性脑梗死患者Apo Eε4携带者NIHSS评分高于非携带者的NIHSS评分(P<0.05)。结论 Apo Eε2可能为保护因子,而Apo Eε4可能为脑梗死的易感因子;联合检测Apo E基因型、血清CRP含量和NIHSS评分,对判断脑梗死预后有一定的意义。
Objective To investigate the relationship between polymorphism of Apo E gene and CRP or NIHSS in cerebral infarction.Methods 108 patients with cerebral infarction(70 cases in acute cerebral infarction,38 cases after acute cerebral infarction)and 76 healthy control group were enrolled.Polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)was used to determine Apo E genotype.The total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL-C),high density lipoprotein(HDL-C)and C-reactive prorein(CRP)levels were tested with the automatic biochemical analyzer.NIHSS scores in patients with cerebral infarction were scaled within admission,one week and two weeks after admission.Results In the patients with cerebral infarction,the frequency of Apo E allele ε2 and ε4 account for 6.9% and 9.7% respectively,while the frequency of Apo E allele ε2 and ε4 account for 19.1% and 3.9% respectively in the healthy control group(P〈0.05).The TC,TG,LDL-C levels in cerebral infarction group were significantly higher than that of control group while The HDL-C levels in cerebral infarction group were significantly lower than that of control group(P〈0.05).In patients with acute cerebral infarction,CRP level with ε4 alleles was increased significantly compared with that of without Apo E ε4(P〈0.05).The scores of NIHSS with ε4 alleles was significantly increased without ε4(P〈0.05).In patients with acute cerebral infarction,the scores of NIHSS with ε2 alleles was significantly declined compared with that of without Apo E ε2,while the scores of NIHSS with Apo E ε4 alleles was increased without Apo E ε4(P〈0.05).Conclusion The Apo E ε4 might be a susceptible factor and ε2 be a protective factor for cerebral infarction respectively.Examination of Apo E genotype,serum CRP level and NIHSS score may be helpful for prediction of actue cerebral infarction.
