摘要
目的:研究原发性高血压病患者环氧化酶-2(COX-2)基因多态性的分布及其与高血压病中医辨证分型的关系,为中医辨证论治提供新的客观依据。方法:选择178例高血压病患者,辨证分型为肝火亢盛型、阴虚阳亢型、痰湿壅盛型、阴阳两虚型,并设725例正常者为对照组。运用PCR-RFLP技术检测受试者外周血细胞中COX-2基因启动子区-1290A/G、-1195G/A、-765G/C位点基因多态性,并进行基因分型;分析不同中医证型与COX-2基因表达的相关性。结果:COX-765G/C基因在实验组与正常对照组的分布存在差异;COX-1195G/A基因多态性与高血压肝火亢盛证、阴虚阳亢证的发生有相关性,与虚证(阴虚阳亢+阴阳两虚)比较,实证(肝火亢盛+痰湿壅盛)GG、CG+CC基因型频率明显增高。结论:COX-1195G/A基因变异可能是高血压肝火亢盛证、阴虚阳亢证发生过程的重要遗传因素;-765G/C位点GG、CG+CC基因频率可能与高血压病虚实辨证相关。
Objective: To observe the correlativity between patterns of hypertension in TCM and distribution of COX-2 poly- morphism, and try to provide a new objective evidence for pattern identification. Methods: One hundred and seventy-eight cases with hypertension were divided into four patterns : hyperactivity of liver fire pattern, hyperactivity of yang due tn asthenia of yin pattern, exuberance of phlegm-damp pattern and asthenia of both yin and yang pattern. Seven hundred and twenty-five healthy pc,pie were in normal control group. The gene polymorphisms on loci-1290A/G, -1195G/A, and -765G/C which located in COX-2 gene promoter of peripheral blood cells were detected by polymerase chain reaction-restrlction fragment length polymorphism ( PCR- RFLP). The genes were divided into different patterns, and the co^a'elativity between patterns and expression of COX-2 was ana- lyzed. Results: There was a statistical difference in the distribution of COX-765G/C gene between experimental group and control gronp ; the~ was correlativlty between polymorphism of COX-1195G/A and occm'rence of hyperactivity of liver fire pattern and hy- peractivity of yang due to asthenia of yin pattern. The genotype frequency of GG and CG + CC in excess patterns( hyperactivity of liver fire, exuberance of phlegm-damp) was increased obviously than in asthenia patterns( hyperactivity of yang due to asthenia of yin, asthenia of bnth yin and yang). Conclusion : The genovariation of COX-1195G/A may be the important factor in hyperactivity nf liver fire pattern amt hyperactivity of yang due to asthenla of yin pattern; the GG and CG + CC gene frequeney on loci-765G/C may he related to the pattern identification of asthenia and asthenia in hypertension.
