摘要
目的探讨慢性肾功能不全患者血与尿中视黄醇结合蛋白的变化,明确血与尿中RBP的相关性及其临床意义。方法分别采用过氧化物酶偶联法和免疫散射比浊法检测慢性肾功能不全患者和健康对照组SCr、血RBP与尿RBP的含量。结果慢性肾功能不全和健康对照组的血清肌酐、RBP和尿RBP水平分别为:Ⅰ期:150.2 mol/L±14.9 mol/L,63.2 mg/L±18.6 mg/L,7.7 mg/L±3.8 mg/L;Ⅱ期:243.2 mmol/L±63.9 mmol/L,74.3 mg/L±27.9 mg/L,12.3 mg/L±9.6 mg/L;Ⅲ期:552.5 mol/L±71.6 mol/L,103.9 mg/L±7.5 mg/L,11.6 mg/L±6.2 mg/L;Ⅳ期:899.6 mol/L±72.0 mol/L,133.3 mg/L±17.0 mg/L,6.1 mg/L±2.8 mg/L;健康对照组:60.9 mol/L±2.3 mol/L,47.0 mg/L±4.8 mg/L,0.22 mg/L±0.08 mg/L]。与健康对照组相比,4个时期的慢性肾功能不全患者血清和尿中的RBP水平显著升高(P<0.05),而尿RBP的水平变化更为明显(P<0.01),从相关性看,血清RBP和尿RBP的含量表现为低相关性(r=-0.221,P>0.05),但血清RBP与血清肌酐表现出显著的相关性(r=0.784,P<0.05)。结论血与尿的RBP的检查和结果分析,对辅助诊断慢性肾功能不全具有重要的参考意义,而且尿RBP的敏感性更好。
Objective To investigate the changes of chronic renal insufficiency in patients with blood and urine retinol binding protein.Clear blood and urine RBP and its clinical significance. Methods using the peroxidase coupling method and immune nephelometry detected in patients with chronic renal insufficiency and healthy control group of SCr,the content of the blood RBP and urinary RBP. Results Chronic renal insufficiency and healthy control group,serum creatinine,of RBP and urinary RBP levels as [a:(150.2±14.9)μmol/L(63.2±18.6)mg/L(7.7±3.8)mg/L;two:(243.2±63.9)μmol/L,and(74.3±27.9)mg/L(12.3±9.6)mg/L;three:(552.5±71.6)μmol/L,and(103.9±7.5)mg/L,and(11.6±6.2)mg/L;four:(899.6±72.0)μmol/L,(133.3±17.0)mg/L,and healthy controls(6.1±2.8)mg/L,group:(60.9±2.3)μmol/L(47.0±4.8)mg/L,0.22±0.08)mg/L,Compared with the healthy control group,the four stages of chronic renal insufficiency in patients with serum and urine RBP level was significantly higher(P0.05),and urinary RBP levels significantly(P0.01),serum RBP and urinary RBP content was low correlation(r=-0.221,P0.05),serum RBP and serum creatinine showed a significant correlation(r=0.784,P0.05). Conclusion Blood and urine RBP of chronic renal insufficiency has important reference value,and the sensitivity of the urinary RBP was better.
