结直肠癌微卫星不稳定性检测的临床病理意义及方法 被引量:5

摘要 结直肠癌微卫星不稳定性(MSI)检测具有多重临床病理意义。MSI是指DNA甲基化或基因突变致错配修复基因缺失,从而导致微卫星重复序列长度的改变。散发性MSI结直肠癌多位于近端结肠,易伴发肠内及肠外其他器官多发性肿瘤,预后较好但对化疗药物5-氟尿嘧啶疗效欠佳。同时,MSI检测可用于诊断Lynch综合征。此外,免疫组化法也可作为检测肿瘤细胞中错配修复蛋白缺失的一种辅助方法。MSI检测是结直肠癌分子分型的基础,可以从分子遗传学角度加深对结直肠癌及癌前病变的认识了解。本文对结直肠癌MSI检测的临床病理意义、检测指征及检测方法进行简要介绍,以促进这一检查技术在临床及病理学实践中的应用。
