摘要
目的研究炎性细胞因子与不同类型老年性痴呆患者认知功能障碍的关联性。方法选择我院门诊符合DSM-Ⅳ诊断标准的老年痴呆患者75例[其中阿尔茨海默病(AD)组41例,血管性痴呆(VD)组34例]以及正常对照组(HC组)32人,分别检测血清IL-1β、IL-6、TNF-α及CRP浓度,并进行韦氏智力测定(WAIS),比较三组间细胞因子浓度和认知功能的差异,并对两者进行相关分析。结果 AD和VD组与HC组受教育年限有统计学差异(P<0.01),三组血清TNF-α和CRP浓度存在统计学差异(P<0.01和P<0.05),两两比较发现AD组TNF-α浓度与另两组均有统计学差异(P=0.000和P=0.000),VD组与HC组TNF-α浓度差异接近统计学意义(P=0.050)。AD组和VD组MMSE评分、言语智商、操作智商和总智商与正常对照组比较均有统计学差异(P=0.000)。TNF-α浓度与MMSE评分、总智商间呈负相关(r=-0.358和-0.326,P=0.001和0.001)。结论不同类型老年性痴呆患者的血清TNF-α和CRP浓度存在统计学差异,TNF-α浓度可能是反映痴呆严重程度的状态指标。
Objective To investigate the correlation between inflammatory cytokines and cognition disorders in different type of senile dementia patients.Methods 75 senile dementia outpatients according to DSM-Ⅳdiagnostic criteria were enrolled including 41 AD patients and 34 VD patients.32 healthy controls(HC)were also enrolled.All the subjects finished the WAIS test and their serum concentration of IL-1β,IL-6,TNF-α and CRP were also tested.The differences of cytokine concentration and cognitive function among 3 groups were compared and correlation analysis between them was also made.Results There was a significant difference on education time among AD,VD and HC(P0.01).There was also a significant difference on TNF-α and CRP concentration among the three groups(P0.01 and P0.05).TNF-α in group AD was significantly different from those in the other two groups(P=0.000 and P=0.000).There was almost a significant difference of TNF-α concentration between Group VD and Group HC(P=0.050).There were significant differences on Speech IQ,Manipulation IQ and General IQ among the three groups(P=0.000).There was a significant negative correlation between TNF-α concentration and MMSE scores,General IQ(r=-0.358 and-0.326,P=0.001 and 0.001).Conclusions There are significant differences of serum TNF-α and CRP concentration in different type of senile dementia patients,TNF-α concentration might be a state mark of dementia severity.
