减重手术对正常大鼠血糖代谢影响的实验研究

摘要 目的观察不同减重手术对正常大鼠血糖代谢的影响。方法将体重为210-270g的健康雄性Wistar大鼠20只,随机分为4组,每组5只。第1组:行远端胃大部切除、胃空肠Roux-en-Y吻合术(Roux-en-Y Gastric Bypass,RYGB);第2组:行十二指肠旷置、胃与近端空肠吻合保持胃容积的十二指肠空肠转流术(Duodenal-jejunal Bypass,DJB);第3组:行袖状胃切除术(Sleeve Gastrectomy,SG);第4组:假手术组(Shamoperation,SO)。观察各组大鼠术前及术后第1、2、3、4、5周空腹血糖(Fasting Blood Glucose,FBG)变化及术前、术后第3、5周口服葡萄糖耐量试验(Oral Glucose Tolerance Test,OGTT)变化;术后空腹血胰岛素、胰高血糖素样肽-1(GLP-1)及灌胃后15min血胰岛素、胰高血糖素样肽-1(GLP-1)浓度变化情况。结果术后各组大鼠FBG较术前均未发生明显变化(P﹥0.05)。术后第3周各组OGTT未发生明显变化,术后第5周,RYGB组术后OGTT曲线下面积(areas under curves,AUC)较SO组减少17.92%,差异有统计学意义(P<0.05)。该组OGTT60min血糖值较SO组下降,差异有统计学意义(P﹤0.05)。术后3周,各组大鼠空腹或葡萄糖灌胃后15min血胰岛素及胰高血糖素样肽-1浓度与假手术组相比,差异无统计学意义(P>0.05)。术后5周,各组大鼠空腹血胰岛素及胰高血糖素样肽-1浓度与SO组相比,差异无统计学意义(P>0.05);葡萄糖灌胃后15min,RYGB组大鼠血胰岛素浓度较SO组显著升高,差异有统计学意义(P<0.05),此时RYGB、DJB两组大鼠GLP-1浓度较SO组显著升高,差异有统计学意义(P<0.05)。结论 RYGB手术增加餐后血胰岛素分泌,加速餐后血糖代谢。
