摘要 目的探索创建基于股份制的民营社区卫生服务管理模式,并评价其运行效果。方法自2004年起,广州文冲船厂职工医院改制为黄埔区红山街社区卫生服务中心。通过整合卫生资源、改造服务环境、转变服务理念、完善服务模式、强化卫生队伍建设、落实内涵建设等方法,探索构建了股份制民营社区卫生服务管理模式。采用定量研究方法对该社区卫生服务管理模式的效果效益进行评价。结果红山街社区卫生服务中心已经成功构建了基于股份制的民营社区卫生服务管理模式,并取得了一定成效。近年来,红山街社区卫生服务中心的人力资源总数量和质量呈现逐年上升的趋势。2006—2010年,红山街社区卫生服务中心的业务量、公共卫生服务的数量和质量、居民和中心员工满意度均逐年上升,其中2010年的居民健康行为形成率、健康知识知晓率、满意度和员工满意度分别为76.0%、84.0%、94.0%和85.0%。在效益方面,中心近年的总体收支平衡,略有盈余,而门诊人均处方费用呈逐年递减趋势。结论红山街社区卫生服务中心的股份制民营社区卫生服务管理模式效果良好,为企业医院转型的社区卫生服务机构提供了科学的参考。 Objective To explore how to establish a private model of community health services based on shareholding system, and to assess its efficacy of operation. Methods The worker's hospital of Wenchong shipyard had been reformed and constructed to be a community health services (CHS) center of private shareholding model in Hongshanjie community since 2004. The measures included were integration of health resource, improving service circumstance, changing service philosophy, impro- ving service model, enhancing the establishment of health contingent, and implementing the construction of connotation. Quanti- tative research methods was used to evaluate the overall situation of the CHS before and after the implementation of the new mod- el. Results Private shareholding model of CHS management was established successfully, and a certain achievement had been gained in Hongshanjie community. Evaluation showed that there were' an increasing trend on the total quantity and quality of hu- man resources. From 2006 to 2010 the quantity of business, quantity and quality of the public health services, and the satisfac- tion of the residents and the CHS center's staff had been increasing year by year. In 2010 the forming rate of health behavior, awareness rate of health knowledge, and satisfaction of community residents and the medical staff' s satisfaction were 76.0%, 84.0%, 94. 0%, and 85.0%, respectively. In recent years a balance was generally kept between income and expenditure, with a slight surplus. The prescription cost per outpatient was decreasing year by year. Conclusion The effect of private share- holding model of community health service is satisfactory, which gives a scientific reference for the enterprise hospital to change its operation model.
