阔筋膜张肌重建臀中肌功能的临床研究 被引量:4

摘要 目的总结人工全髋关节置换术中采用阔筋膜张肌重建毁损臀中肌的方法及疗效。方法 2007年6月及2009年12月,收治2例臀中肌毁损女性患者,年龄分别为55岁及62岁。骨巨细胞瘤1例,髋部骨折1例;均为在外院术中毁损臀中肌及股骨粗隆部。末次术后至此次入院时间分别为14 d和18 d。患侧髋关节功能Merle D’Aubigne评分分别为5分及6分。采用定制人工全髋关节行全髋关节置换并阔筋膜张肌重建臀中肌功能。结果患者术后切口均Ⅰ期愈合;1例术后15 d发生髋关节脱位,采用手法闭合复位并加强功能锻炼,未再出现脱位。患者术后分别获随访18个月及24个月。末次随访时患肢均短缩<2 cm。按Merle D’Aubigne评分法评分分别达17分和15分。结论人工全髋关节置换术中采用阔筋膜张肌重建毁损臀中肌,可最大程度重建髋关节周围的软组织生理解剖结构,提高髋关节周围软组织平衡度和关节稳定性,改善术后患髋功能。
