摘要
目的:观察锁定加压钢板内固定治疗Rüedi-AllgwerⅢ型Pilon骨折的手术方法及临床疗效。方法:2007年1月-2010年1月,采用开放复位、锁定加压钢板内固定治疗闭合性Rüedi-AllgwerⅢ型Pilon骨折25例,其中男16例,女9例;年龄19～65岁,平均38岁。术后随访时间12～24个月,平均18个月,根据Mazur踝关节症状与功能评分系统进行临床疗效评定。结果:优17例,良4例,可3例,差1例,优良率84%。无1例发生皮瓣坏死。无螺丝钉松动、断裂及内固定失败发生。结论:锁定加压钢板内固定治疗Rüedi-AllgwerⅢ型Pilon骨折复位满意,固定牢靠,操作简单,创伤较少,具有疗效佳、康复快、功能好等优点,是手术治疗Rüedi-AllgwerⅢ型Pilon骨折安全、有效的内固定方法之一。
Objective：To report the operation and clinical effects of locking compression plate in the treatment of Rüedi-Allg？wer type Ⅲ Pilon fracture.Methods：From January 2007 to January 2010,25 patients with closed Rüedi-Allg？wer type Ⅲ Pilon fracture,including 16 males and 9 females,were treated with surgical reduction and internal fixation with locking compression plate.The mean age of the patients was 38 years（19-65 years）.The follow-up period ranged from 12 to 24 months with a mean of 18 months.The clinical results were assessed according to the Mazur ankle symptoms and functional scoring system.Results：The effects were excellent in 17 cases,good in 4,fair in 3 and poor in 1 case.The satisfactory rate was 84%.No skin flap necrosis occurred.No screw sliding,dislodging or fixation failure was found during follow-up.Conclusion：In treatment of Rüedi-Allg？wer type Ⅲ Pilon fracture,locking compression plate not only maintains satisfying reduction with reliable fixation,but also manipulates simply and invades slightly,thus gets good curative effect,fast recovery,satisfying function and so on.So it is one of the effective fixation techniques for Rüedi-Allg？wer type Ⅲ Pilon fracture.
