2型糖尿病周围神经病变与代谢综合征相关性探讨 被引量:6

A Study on Relationship of Metabolic Syndrome with Diabetic Peripheral Neuropathy in Patients with Type 2 Diabetes

摘要 目的:探讨2型糖尿病合并糖尿病周围神经病变与代谢综合征(MS)的相关性。方法:对302例2型糖尿病患者进行Michigan糖尿病神经病变症状评分和Michigan体征评分及神经电生理检查,同时进行代谢综合征组分(腰围、血糖、血脂、血压)测定,比较上述指标在MS组与非MS组间的差异。结果:糖尿病合并MS组较非MS组周围神经患病率(57.5%vs44.3%)、神经症状评分(4.12±1.89vs1.69±1.54)和体征评分(2.15±1.60vs1.56±1.01)增高,差异有统计学意义(P<0.05),糖尿病合并MS组较非MS组左腓感觉神经传导速度[(43.7±4.6)m/svs(46.1±4.0)m/s]、右腓感觉神经传导速度[(40.0±4.9)m/svs(45.9±4.5)m/s]及左正中感觉神经传导速度[(51.7±4.2)m/svs(55.5±4.3)m/s]减慢,差异有统计学意义(P<0.05)。多因素回归分析显示,糖尿病病程长、腰围大、收缩压高、糖化血红蛋白及尿微量白蛋白排泄率高是糖尿病神经病变发生的危险因素。结论:MS与2型糖尿病患者周围神经病变的发生密切相关,它的大多数组分是糖尿病神经病变的危险因素。 Objective: To investigate the relationship between peripheral neuropathy and metabolic syndrome (MS) in patients with type 2 diabetes. Methods: Three hundred and two patients with type 2 diabetes were detected by Michigan diabetic neuropathy score and nerve eleetrophysiologieal test. Patients were divided into 2 groups (MS group and non-MS group) according to their waistlines, blood glucose, blood lipids and blood pressure. Results: The prevalence of peripheral neuropathy (57.5%), Michigan diabetic neuropathy symptom score (4.12± 1.89) and sign score (2.15±1.60) were significantly higher in MS group than those of non-MS group (44.3%, 1.69±1.54 and 1.56±1.01, P 〈 0.05). There was a significantly lower peroneal sensory nerve conduction velocity (43.7 m/s ±4.6 m/s in left, 40.0 m/s±4.9 m/s in right) in MS group than that in non-MS group (46.1 m/s±4.0 m/s in left,45.9 m/s ±4.5 m/s in right).The left median sensory nerve conduction velocity was also significantly lower in MS group (51.7 m/s±4.2 m/s) than that of non-MS group (55.5 m/s±4.3 m/s, P 〈 0.05). Multiple regression analysis showed that the disease duration, waist circumstance, systolic blood pressure, glycosylated hemoglobin and urinary albumin excretion rate were independently related to peripheral neuropathy (P 〈 0.05). Conclusion: Metabolic syndrome may be related to the progress of diabetic peripheral neuropathy in patients with type 2 diabetes, and its most components were independent risk factors of diabetic peripheral neuropathy.
