尿白蛋白正常的2型糖尿病患者肾功能与尿β2微球蛋白关系的探讨

摘要 目的分析尿白蛋白正常的2型糖尿病患者肾功能与尿β2微球蛋白的关系。方法 24h-UAE<30 mg的2型糖尿病患者共205例,以改良的MDRD公式计算eGFR,同时检测尿β2微球蛋白,分析两者间的相关性。结果 205例正常白蛋白尿的2型糖尿病患者,38例(18.54%)肾功能下降。患者尿β2微球蛋白与eGFR、年龄、血肌酐、血尿酸水平显著负相关(r=-0.499,-0.225,-0.541,-0.232,均P<0.01);与SBP呈负相关(r=-0.162,P<0.05)。结论不少2型糖尿病患者24 h尿白蛋白正常,但肾功能已经下降,其肾功能损害与尿β2-MG密切相关。临床上在检测24 h尿白蛋白的基础上,应同时检测尿β2-MG、评估肾小球滤过率,以利于糖尿病肾病的早期诊治。 Objective To invesgate the relationship between renal function, marly glomerular filtration and urine β2-mi- croglobulin in normoalbuminuric type 2 diabetic patients. Methods Two hundred and five normoalbuminuric Type 2 dia- betic patients were included in the study. We used simplified equation of MDRD to estimate their GFR, and examine their urine β2-microglobulin at the same time. We used Person regression to assess variables independently associated withβ2- microglobulin. Results In the 205 normoalbuminuric type 2 diabetic patients, 38 cases (18.54%) with low glomerular filtration. The urine β2-microglobulin is significantly related to eGFR decline, age, the level of serum creatinine and uric acid (r = - 0.499, --0.225, --0.541, --0.232, all P 〈 0.01), also it was related to systolic blood pressure (r = -0.162, P 〈 0.05). Conclusion A con siderable proportion of the type 2 diabetic patients have low glomerular filtration when their urinary albumin excretion are still normal. The urineβ2-microglobulin maybe related to the low glomerular filtration. It is suggested that urine β2-mi- croglobulin and eGFR could benefit the early detection of renal disease in such patients.
