经皮肾镜碎石取石术1572例报告及术后感染情况分析 被引量:4

摘要 目的总结经皮肾镜碎石取石术(PCNL)治疗经验,探讨PCNL术后感染的处理方法及预防措施。方法收集采用PCNL治疗的1 572例肾结石患者的临床资料,进行回顾性分析,患者共行1 711例次PCNL,其中139例因术后结石残留行二次手术。结果术后发生全身炎症反应综合征(SIRS)441例次(25.8%),发生感染性休克8例(0.5%)。经早期抗感染、抗休克治疗均好转。结论术后感染是PCNL术后常见并发症,要充分重视并早期采取积极防治措施,术前常规留置中段尿培养和进行药物敏感试验,术中采用合适的工作通道,术后常规对感染性结石进行细菌培养及药敏试验,有利于避免严重的不良事件发生。 Objective To summarize the experience of percutaneous nephrolithotomy (PCNL) and to evaluate the treat- ment and prevention of postoperative infection. Methods Records of 1 572 patients who underwent PCNL were analyzed retro- spectively. The frequency of PCNL was 1 711. A total of 139 patients suffered second operation. Results After PCNL, the frequency of SIRS was 441 (25.8%) and eight cases (0.5%) were identified with a diagnosis of septic shock. The prophase antiqnfection and anti-shock treatment was effective. Conclusion Postoperative infection is common complication after PCNL. The prophase efficacious therapy should be emphasized to avoid the occurrence of more adverse events.
