益气温阳方对心梗后心力衰竭大鼠心功能和心肌磷酸腺苷含量的影响 被引量:4

Effect of "Yiqi Wenyang Decoction" on cardiac function and content of ATP in rats with heart failure after myocardial infarction

摘要 目的研究益气温阳方对心梗后心衰大鼠心功能和心肌磷酸腺苷含量的影响。方法采用结扎冠状动脉前降支术制备心梗后心衰大鼠模型。手术2周后将大鼠分为模型组、假手术组、曲美他嗪组、益气温阳低剂量组、益气温阳中剂量组、益气温阳高剂量组,每组10只,并给予相应的干预措施,共8周。采用酶联免疫吸附法检测心梗后心力衰竭大鼠药物干预前后脑钠肽(BNP)水平,测定治疗后血流动力学指标,包括心率(HR)、左室收缩压(LVSP)、左室舒张压(LVEDP)、左室内压上升下降最大速率(±LVdp/dtmax),并用高效液相法测定心肌组织中AMP、ADP、ATP的含量。结果心功能方面,与模型组比较,治疗后,假手术组、曲美他嗪组和益气温阳中、高剂量组大鼠LVSP、+LVdp/dtmax升高(P<0.05,P<0.01),LVEDP、-LVdp/dtmax和BNP降低(P<0.05,P<0.01);与假手术组比较,曲美他嗪组及益气温阳中、低剂量组大鼠LVSP、+LVdp/dtmax降低(P<0.05,P<0.01),LVEDP、-LVdp/dtmax和BNP升高(P<0.01);与曲美他嗪组比较,益气温阳中、低剂量组大鼠+LVdp/dtmax降低(P<0.05,P<0.01),LVEDP、-LVdp/dtmax升高(P<0.01),其中低剂量组大鼠BNP升高(P<0.01);与益气温阳高剂量比较,益气温阳中、低剂量组大鼠LVSP、+LVdp/dtmax降低(P<0.05,P<0.01),LVEDP、-LVdp/dtmax升高(P<0.01),其中低剂量组大鼠BNP升高(P<0.01)。能量代谢方面,与模型组比较,假手术组、曲美他嗪组及益气温阳中、高剂量组大鼠AMP、ADP升高(P<0.05,P<0.01),ATP降低(P<0.05,P<0.01);与假手术组比较,益气温阳各剂量组大鼠ADP升高(P<0.05,P<0.01),ATP降低(P<0.01);与曲美他嗪组比较,益气温阳中、低剂量组大鼠ADP升高(P<0.05,P<0.01),ATP降低(P<0.01),益气温阳高剂量组差异无统计学意义(P>0.05);与益气温阳高剂量组比较,益气温阳低剂量ATP降低(P<0.01)。结论益气温阳方可改善心衰大鼠的心功能,优化衰竭心肌细胞的能量代谢。 Objective To observe the effect of "Yiqi Wenyang Decoction" on cardiac function and content of ATP in rats with heart failure after myocardial infarc- tion. Methods Anterior descending branch of coronary artery was ligated to establish the rat models. After 2 weeks, the rats were divided into 6 groups : model group, sham-operation group, Trimetazidine group (TMZ group) and "Yiqi Wenyang" groups (large, medium and small dose), 10 rats in each group, and inter- vened for 8 weeks. The BNP level was determined by ELISA, Hemodynamic changes such as HR, LVSP, LVEDP and ( + LVdp/dtmax ) were detected, and the contents of AMP, ADP and ATP were measured by HPLC. Results For the cardiac function, compared with model group after treatment, the LVSP and + LVdp/ dtmax were increased in sham-operation group, TMZ group, large dosage and medium dosage of "Yiqi Wenyang" groups ( P 〈 0.05, P 〈 0.01 ), while the LV- EDP, -LVdp/dtm~ and BNP were decreased (P 〈 0.05, P 〈 0.01 ) ; compared with the sham-operation group, the LVSP and + LVdp/dtm~ were reduced in TMZ group, medium dosage and small dosage of "Yiqi Wenyang" groups ( P 〈 0.05, P 〈 0.01 ) ; while the LVEDP, -LVdp/dtm^x and BNP were increased ( P 〈 0.01 ) ; compared with TMZ group, the + LVdp/dtmax was reduced in medium dosage and small dosage of "Yiqi Wenyang" groups (P 〈 0.05, P 〈 0.01 ), while the LVEDP and -LVdp/dtmax were increased '( P 〈 0.01 ), the BNP in small dosage of "Yiqi Wenyang" group was increased ( P 〈 0.01 ) ; compared with large dosage of "Yiqi Wenyang" group, the LVSP and + LVdp/dtmx in medium and small dosage of "Yiqi Wenyang" groups were reduced ( P 〈 0.05, P 〈 0.01 ), while the LVEDP and -LVdp/dmax were increased ( P 〈 0.01 ), the BNP in small dosage of "Yiqi Wenyang" group was increased (P 〈 0.01 ). For energy metabolism, compared with model group, the AMP and ADP were increased in sham-operation group, TMZ group, medium and large dosage of "Yiqi Wenyang" groups (P 〈 0.05, P 〈 0.01 ) , the ATP was decreased (P 〈 0.05, P 〈 0.01 ) ; compared with the sham-operation group, the ADP was increased in three "Yiqi Wen- yang" groups ( P 〈 0.05, P 〈 0.01 ), while the ATP was decreased ( P 〈 0.01 ) ; compared with TMZ group, the ADP in medium and small dosage of "Yiqi Wen- yang" groups was increased (P 〈 0.05, P 〈 0.01) , and the ATP was decreased (P 〈0.01 ) , without significant difference from large dose of "Yiqi Wenyang" group ( P 〉 0.05 ) ; compared with large dosage of "Yiqi Wenyang" group, the ATP in small dosage group was decreased ( P 〈 0.01 ). Conclusion "Yiqi Wen- yang Decoction" can improve the cardiac function in rats with heart failure and optimize the energy metabolism of the myocardical cells.
