摘要
目的分析比较高强度聚焦超声(HIFU)消融联合同步吉西他滨与单纯吉西他滨化疗治疗对中晚期胰腺癌的治疗效果。方法中晚期胰腺癌患者56例,随机分为联合组26例和化疗组30例,分别接受HIFU消融同步吉西他滨化疗及单纯吉西他滨化疗。观察近期疗效、临床收益率、吉西他滨化疗剂量及不良反应。结果治疗后1~3个月MRI复查评价疗效,联合组肿瘤瘤体坏死率明显高于化疗组(88.5%vs43.3%,P<0.01);治疗后1个月临床收益反应评价,联合组临床收益率明显高于化疗组(80.0%vs30.8%,P<0.05)。吉西他滨用量[(550±230)vs(658±100)mg/m2,P<0.01]及不良反应分级[(2.0±0.3)vs(2.2±0.6)级,P<0.05]联合组均明显低于化疗组。2组均未发生严重不良反应及并发症。结论胰腺癌HIFU消融联合同步吉西他滨化疗,具有较好的近期疗效,且毒性反应轻,不良反应可耐受,为中晚期胰腺癌提供了一种新的治疗方法。
Objective To compare the therapeutic effects of high intensity focused ultrasound (HIFU) ablation combined with concurrent gemeitabine chemotherapy for mid-term and advanced pancreatic cancer. Methods Fifty-six patients with mid-term and advanced pancreatic cancer were randomly divided into combination group (n = 26) and chemother- apy group ( n = 30). HIFU combined with gemcitabine chemotherapy was applied in combination group, and gemeitabine chemotherapy alone was administered in chemotherapy group. Short-term therapeutic effect, clinical benefit rate ( CBR), gemcitabine chemotherapy dose and adverse reactions were observed. Results Tumor entity necrosis rate in combination group was significantly higher than that in chemotherapy group (88.5 % vs 43.3 % , P 〈 0. 01 ) during MRI reexamination 1- 3 months after treatment. CBR in combination group was significantly higher than that in chemotherapy group (80.0% vs 30.8%, P 〈0.05) 1 month after treatment. Gemeitabine dose and adverse grade in combination group were all lower than those in chemotherapy group [ ( 550 + 230) vs ( 658 + 100) mg/m2, P 〈0. 01 ; ( 2.0 + 0.3 ) vs ( 2.2 + 0.6), P 〈 0.05 ]. No serious adverse reactions and complications occurred in two groups. Conclusions Compared with chemotherapy alone, HIFU ablation combined with concurrent gemeitabine chemotherapy can gain better short-term efficacy and has advantages of lighter toxicity and tolerable adverse effects, therefore it provides a new, safe and effective therapeutic method for mid-term and advanced pancreatic cancer.
