摘要
目的观察单纯水平肌手术后共同性水平斜视伴轻度垂直斜视患者的垂直眼位的变化情况。方法收集2010年1至12月的住院患者共19例。术前水平斜视伴轻度垂直斜视（≤10^△）的患者，眼底照相未见明显旋转斜视，手术只行水平肌后徙和缩短手术，未做水平肌的垂直移位，未行垂直肌手术。随访3—12月，观察垂直眼位的变化。结果（1）内斜视伴垂直斜视者7例，术前垂直斜视角4～10^△。平均（7．10±02．15）^△.术后垂直斜视角0～4^△，平均（3．67±1．25）^△，与术前比，降低2^△～5^△，平均降低（4．01±1．00）^△。术后垂直斜视角比术前有降低者6例，占85．71％。手术前后的垂直斜视角比较，t=6．928，P=0．020。（2）外斜视伴垂直斜视12例，术前垂直斜视角3—10^△，平均（6．91±3．09）^△。术后垂直斜视角0-5^△，平均（2．36±3．64）^△，垂直斜视角减少1^△-7^△，平均（4．18±2，86）^△。术后垂直斜视角比术前有降低者10例，占83．33％。手术前后的垂直斜视角比较，t=2．371，P=0．039。（3）垂直斜视角的减少量与术前垂直斜视角的大小成正相关，χ^2=0．023，相关系数pearson cotrelation=0．675。结论水平斜视伴有小度数垂直斜视的患者，单纯水平肌手术后对垂直眼位有一定改善，且术前垂直斜视角越大，术后垂直斜视角的减少越明显。
Objective To investigate the vertical eye position changes after horizontal rectus surgery in concomitant horizontal strabismus with small angle of vertical strabismus. Methods During January to December 2010, 19 cases of horizontal strabismus with small angle of vertical strabismus accepted only medial rectus and lateral rectus surgery, without vertical muscles surgery or vertically transfer of the horizontal rectus. Followed up for 3-6 months, and investigated the vertical eye position changes after operations. Results There were 7 cases of esotropia with vertical strabismus, 6 （85.71%） cases obtained less post-operation-angle-of-vertical-strabismus than pre-operation-angle-of-vertical-strabismus. The pre-operation-angle-of-strabismus was 4-10^△ （7.10^△±2.15^△）, the post-operation-angle-of-strabismus was 2-4^△ （3.67±1.25^△）, reduced 2^△-5^△ （4.01＋1.00^△）. The comparison of angle-of-strabismus showed significant difference pre- and post-operation （P 〈0.05）. There were 12 cases of exotropia with vertical strabismus; 10 （83.33%） cases got less post-operation-angle-of-strabismus than pre-operation-angle-of-vertical-strabismus. The pre-operation-angle-of- vertical-strabismus was 3-10 ^△ （6.91±4-3.095）, the post-operation-angle-of-strabismus was 0-5^△ （2.36^△±3.64^△）, reduced 1^△-7^△ （4.18± 2.86^△）. The comparison of angle-of-strabismus showed significant difference pre- and post-operation （P 〈0.05）. There was positive correlation between the decrement-of-vertical-strabismus and the pre-op- eration-angle-of-vertical-strabismus, χ^2=0.023, pearson correlation=0.675. Conclusions Horizontal rec-ms strabismus correction surgery can reduce the vertical strabismus in concomitant horizontal strabismus with small angle of vertical strabismus.
