摘要
目的:定量揭示胸水中腺癌细胞(adenocarcinoma cell,ACC)的形态结构特征,筛选对ACC诊断分类有重要价值的尺寸和形状参数。方法:取胸水涂片中764个ACC(44例)和709个反应性间皮细胞(reactive mesothelial cell,RMC;39例),用计算机图像分析技术测试细胞和细胞核的尺寸和形状参数;其中尺寸参数包含面积(A)、长轴(dmaj)、短轴(dmin)、平均直径()、周长(C),形状参数包括规化形状因子(RFF)、形状不规则指数(FII)、核浆比(Rnp),对测试结果进行独立样本t检验和逐步判别分析。结果:与RMC相比,ACC的尺寸参数、FII、Rn p明显增大,而RFF明显减小(P<0.05)。逐步判别分析显示,dm a j,c、dm a j,n、An、Dn、Cn、FIIc进入判别函数,其符合率为75.9%。结论:形态结构参数dmaj,c、dmaj,n、An、n、Cn、FIIc对于胸水ACC的判别研究可能有价值。
Objective: To quantitatively anzlyze the morphological structure characteristics of ade- nocarcinoma cell (ACC) in pleural effusion, and to find dimension and form parameters which can be applied to identify ACC. Methods: A total of 764 ACC of 44 cases and 709 reactive mesothelial cells (RMC) of 39 cases were collected from pleural effusion smears. A technique of computer image analysis was applied to test the dimension and form parameters. Dimension parameters include Area(A), Major axis (dmaj), Minor axis(dmin), Mean diameter(D), Circumference(C) ;and form parameters include Regular form factor(RFF), Form irregular index(FII), Ratio of nucleus to cytoplasm(R,p). Results were compared and put through independent-sample t test and stepwise discriminant analysis. Results: Compared with RMC, ACC showed significantly increased values of dimension parameters,FII, Rnp,while decreased RFF (P〈0.05). Stepwise discriminant analysis showed dmaj、dmaj,n、An、Dn、Cn、FIIc entered discriminant function, and its coincidence rate was 75. 9%. Conclusions: Morphological structure parameters of dmaj、dmaj,n、An、Dn、Cn、FIIc may have important value in discriminant study on ACC of pleural effusion.
