摘要
目的探讨我国中年人群腰围身高比值(WHtR)诊断中心性肥胖的适宜切点。方法选用国家"八五"和"九五"攻关课题的两次横断面调查,共30 630名35~59岁人群资料,比较不同WHtR切点对高血压、空腹血糖异常、血清总胆固醇升高(高TC)、血清高密度脂蛋白胆固醇降低(低HDL-C)以及危险因素聚集(危险因素个数≥2)的检出效果,并分析各危险因素及其聚集的人群归因危险度。结果预测4种危险因素的WHtR最佳切点范围在0.48~0.52,预测危险因素聚集的WHtR最佳切点在男性、女性及男女合并人群中分别为0.49、0.50及0.49。人群WHtR控制在0.50以下,可防止人群中约2/5的高血压,1/5空腹血糖异常和高TC,1/3的低HDL-C,以及约一半的危险因素聚集。结论 WHtR为0.50可作为我国中年人群中心性肥胖评价的适宜切点。以"腰围不超过身高一半"作为体重控制理念,促进心血管病的预防。
Objective To explore the optimal cut-off value of waist-to-height ratio(WHtR) in detecting the central obesity in Chinese adult population.Methods A total of 30630 participants aged 35-59 from different areas in China's Mainland were surveyed for cardiovascular diseases risk factors in two independent cross-sectional studies that carried out in 1992-1994 and 1998,respectively.The sensitivity,specificity,positive and negative predictive value and Youden′s index for hypertension,abnormal glucose,high serum total cholesterol(high TC),low serum high density lipoprotein cholesterol(low HDL-C) and clustering of risk factors(number≥2) were calculated to evaluate the efficacy of detecting central obesity.The population attributable risk proportion was also used to analyze the public health signification of the optimal cut-off value.Results The optimal cut-off values of WHtR for 4 risk factors(hypertension,abnormal glucose,high TC,low HDL-C) ranged from 0.48~0.52.Moreover,the optimal values to detect clustering of risk factors cluster in male,female and the whole population were 0.49,0.50,and 0.49,respectively.Keeping WHtR less than 0.50 could prevent about 2/5 hypertension,1/5 abnormal glucose and high TC,1/3 low HDL-C,and one half clustering of risk factors.Conclusions WHtR≥0.50 could be the optimal cut-off value for detecting central obesity in Chinese adult population."Keep waist circumference half of your height" should be the target of control central obesity to prevent cardiovascular diseases.
