福州地区健康体检女性宫颈HPV感染基因型检测及分析 被引量:8

摘要 目的分析福州地区健康体检女性人乳头瘤病毒(human papillomavirus,HPV)的亚型分布特点,为宫颈癌防治、HPV疫苗的开发与应用提供借鉴依据。方法对2010年3月~2011年11月来福建第二人民医院体检中心妇科体检的女性2017例,采集宫颈脱落细胞,采用基因芯片技术,进行常见23种HPVDNA亚型(包括18种高危型和5种低危型)的分型检测,分析HPV感染率、亚型、多重感染等特征;并对其中1493例,以5岁为一年龄段,分析HPV感染的年龄分布特点。结果在2017例标本中,共检出HPV阳性632例,检出率为31.33%。高危型以HPV52(6.89%),HPV16(6%),HPV58(5.5%),HPV66(2.77%)和HPV51(2.57%)为常见;低危型以HPV11(4.27%)和HPV6(4.11%)为常见。单一型别感染检出率为17.35%(350/2017),单一高危型感染的比例(13.29%)高于单一低危型感染的比例(4.06%)。多重(2种或2种型别以上)感染检出率为13.98%(282/2017),表现为多重高危感染(7.33%)和多重高低危混合感染(6.64%)。在1493例中,以20-49岁年龄为主,占所有送检标本的93.5%。两个感染率的高峰为20~24岁(7.64%)和35~39岁(6.63%)。结论福州地区HPV感染以高危型为主,前5位的亚型是HPV52,16,58,11和6。多重感染以二重和三重感染最常见,感染高峰在20~24岁。 Objective To investigate the prevalence and genotypes distribution of human papillomavirus (HPV) infection in health-check female cervix in Fuzhou area, so as to provide the strategy for prevention and treatment of cervical cancer in the future. Methods 2 017 females,who visited Center for Health Check of Fujian Second People's Hospital,for gynecological examination,were recruited. From cervical cast-off cells,gene chip technique was used to detect HPV-23 types,including 18 High-risk (HR) type and 5 low-risk (LR) types. The infection rate,genotypes,multiple-types of HPV infection were analyzed. And age distribution of HPV infection were further investigated in 1 493 females, as 5 years old for one age group. Resuits In 2 017 cases, the overall prevalence of HPV infection was 31.33. % (632 /2017). HPV52, the most common HR type,was detected in 6.89% ,followed by HPV16(6%), HPV58 (5.5%), HPV66 (2.77%) and HPVS1 (2.57%). HPV11 (4.27%) and HPV6(4.11%) were the most frequent LR types. In 632 cases of HPV infection,350 cases (17.35%) were single-type infection. The rate of HR was 13.29% (268 cases) ,more than the rate of LR (4.06%, 82 cases) in the single infection. The prevalence of multiple-type infection was 13.98% (282 cases) ,7.33% for multiple HR infection and 6.64% for multiple HR-LR infection,respectively. In 1 493 cases,93.5% of the specimen was from the age groups of 20 to 49 years old. And the prevalence was highest in the age group of 20-24 years old (7.64%) ,followed by the age group of 35-39 years old (6.63%). Conclusion In this survey, HR genotypes were the main types in HPV infection in Fuzhou area. The most prevalent HPV types were HPV52,16,58,11 and 6. Doubleand Triple-types infection were predominated in multiple HPV infection. The peak prevalence was in the age group of 20-24 years old.
