摘要
目的探讨膝关节色素沉着绒毛结节性滑膜炎(PVNS)的磁共振成像(MRI)表现。方法回顾性分析11例经手术及病理检查证实的膝关节PVNS的MRI影像资料。结果 11例膝关节PVNS均为弥漫型。MRI表现为关节腔内滑膜增生形成绒毛结节或肿块,T1WI呈等或低信号,T2WI呈低信号,其中6例在T2WI上部分病灶呈低、高混杂信号;11例均有不同程度关节积液,关节内脂肪垫受侵10例,软骨或骨质受侵8例,关节韧带受侵6例,关节囊肿胀7例,关节周围软组织受推移7例,累及半月板1例。结论 MRI对膝关节PVNS有特征性表现,并敏感显示关节内、外组织结构的侵犯程度及范围,是诊断PVNS的理想检查方法。
Objective To investigate the MRI manifestations of pigmented villonodular synovitis (PVNS) of the knee joint. Methods The MRI image data in 11 cases of knee joint PVNS confirmed by operation and pathology were retrospectively analyzed. Results 11 cases of PVNS were diffuse type. The main MRI manifestations were as follows:the synovial proliferation in the cavity formed villar nodules or mass,which were isointensity or hypointensity on T1WI and hypointensity on T2WI, among them some lesions in 5 cases were shown as heterogeneously hypo/hyper-intensity on T2WI. Various degrees of intra-articular effusion were seen in all cases,invasion of articular fat pad in 10 cases, cartilage and skeletal erosion in 8 cases, destruction of joint ligment in 6 cases and swelling of joint sac in 7 cases. Soft tissue around the joint was displaced in 7 cases. The involvement of the meniscus occurred in 1 case. Conclusion Characteristic manifestations of knee joint PVNS could be showed on MRI,the extent as well as the degree of intra-and extra-articular invasion could also be revealed. MRI is an ideal examination method for the diagnosis of PVNS.
