摘要
[目的]比较微创与常规全髋关节置换术(total hip arthroplasty THA)的短期临床随访结果。[方法]将2008年1月至2011年1月施行THA的200例患者随机分成两组,前外侧入路微创组和标准后外侧入路组,各100例患者,对两组手术时间、围手术期出血量、引流量、切口长度、术后住院时间、并发症、疼痛评分、Harris评分进行比较。[结果]平均随访20个月(12～30个月)。两组患者均无骨折、感染、脱位、血管神经损伤、血栓形成等并发症。两组相比平均手术时间、末次随访VAS评分、平均Harris评分及术后影像学评价,差异无统计学意义(P>0.05);平均切口长度、围手术期出血量、引流量、输血量、平均住院时间,两组差异有统计学意义(P<0.05)。[结论]前外侧入路微创全髋关节置换具有创伤小、出血少、疼痛小、恢复快的特点,是一种安全、有效、可靠的方法。
[ Objective]To retrospectively analyze the clinical results of minimally invasive total hip arthroplasty with an- terolateral incision. [ Method] Two hundred cases were randomly divided into two groups. One hundred cases（ group 1 ）who had undergone a mini-invasive THA were compared with one hundred cases （group 2 ）who had undergone THA with standard pos- terolateral incision. The operative time, perioperative bleeding, drainage volume,length of incision, postoperative hospitalization, complication,postoperative VAS pain score, Harris Hip Score and prosthetic position were analyzed and compared between two groups. [ Result ] All patients were followed up for 12 to 30 months, with the mean time 21 months. No fracture, infection, dislo- cation,neurovascular, symptomatic deep venous thrombosis discovered in two groups. The average operative time, the average postoperative VAS pain score, the average HmTis score, and the prosthesis position had no significant difference between both groups（P 〉 0.05 ）. The average length of incision, the average perioperative blood loss, the average postoperative drainage, the average blood transfusion, and the average length of postoperative hospitalization had significant difference between both groups （P 〈0.01 ）. [ Conclusion] Minimally invasive THA with anterolateral incision has the advantages of less soft tissue damage, less blood loss, less pain and quick recovery, which is a safe and effective technique.
