原发性高血压患者血尿酸水平与冠状动脉病变程度的相关性分析 被引量:8

Correlation between serum uric acid level and severity of coronary artery lesions in patients with essential hypertension

摘要 目的评估原发性高血压患者血尿酸(serum uric acid,SUA)水平与冠状动脉病变程度的相关性。方法收集我院2010年1月至2011年12月行冠脉造影的原发性高血压患者538例,包括男性433例,女性105例,年龄31~84(62.9±9.75)岁,其中明确冠心病362例,排除冠心病176例,根据SUA水平四分位数将原发性高血压患者分为第1四分位数组、第2四分位数组、第3四分位数组、第4四分位数组,分析各组患者SUA水平与冠状动脉病变程度的相关性,冠状动脉病变程度由冠状动脉病变支数及病变Gensini积分表示。结果 SUA是高血压患者合并冠心病的独立危险因素(OR=1.003,95%CI 1.001~1.005,P<0.05),原发性高血压合并冠心病患者SUA水平与冠状动脉病变支数及冠状动脉病变Gensini积分有显著正关联(r=0.128,P<0.01;r=0.163,P<0.01)。结论原发性高血压患者SUA水平升高与冠状动脉病变程度密切相关。 Objective To investigate the correlation between serum uric acid(SUA) level and the severity of coronary artery lesions in patients with essential hypertension.Methods A total of 538 essential hypertension patients who had undergone coronary angiography from Jan.2010 to Dec.2011 in our hospital were enrolled,including 362 patients with coronary heart disease(CHD) and 176 patients without CHD.According to the level of SUA,the essential hypertension patients were divided into the first quartile groups,the second quartile groups,the third quartile groups and the fourth quartile groups.The correlation between the level of SUA and the severity of coronary artery lesions was analyzed.The severity of coronary artery lesions was expressed by the number of stenosed vessels and coronary artery disease Gensini cumulative index.Results The level of SUA was an independent risk factor for essential hypertension patients complicated with CHD(OR=1.003,95%CI 1.001 to 1.005,P0.05),and was positively correlated with the number of stenosed vessels and the coronary artery disease Gensini cumulative index in the essential hypertension patients complicated with CHD(r=0.128,P0.01;r=0.163,P0.01).Conclusion Elevated level of SUA is closely correlated with the occurrence of CHD and the severity of coronary artery lesions in patients with essential hypertension.
