增殖诱导配体检测在B细胞非霍奇金淋巴瘤中的临床意义

摘要 目的:研究增殖诱导配体(APRIL)mRNA及蛋白的表达与B细胞非霍奇金淋巴瘤(B-NHL)患者临床分期、肿瘤负荷及疗效的关系。方法:采用实时荧光定量聚合酶链反应(RFQ-PCR)和酶联免疫吸附试验(ELISA)方法,检测各期B-NHL患者化疗前后外周血单个核细胞和血浆中APRIL mRNA及蛋白含量的变化。结果:化疗前,B-NHLⅢ期及Ⅳ期患者APRIL mRNA及蛋白表达明显高于Ⅰ/Ⅱ期(P<0.05),但Ⅲ期与Ⅳ期之间差异无统计学意义(P>0.05);化疗后,Ⅲ期与Ⅰ/Ⅱ期及Ⅳ期差异无统计学意义(P>0.05),但Ⅳ期明显高于Ⅰ/Ⅱ期(P<0.05)。化疗后B-NHL患者进展(PD)组APRIL mRNA及蛋白表达显著高于完全缓解(CR)及部分缓解(PR)组,与无变化(NC)组差异无统计学意义(P>0.05),且CR、PR及NC组间差异无统计学意义(P>0.05)。结论:APRIL与B-NHL的分期、肿瘤负荷量及疗效密切有关,且可能成为有效治疗B-NHL的新靶点。 Objective:Study on the relationship between the level of a proliferation-inducing ligand(APRIL)mRNA and protein in patients with B cells non-Hodgkin lymphoma(B-NHL)and its clinical stage,tumor burden and efficacy.Method:Real-time fluorescence quantitative polymerase chain reaction(RFQ-PCR)and enzyme-linked immunosorbent assay(ELISA)were used to determine the level of APRIL mRNA and protein in peripheral blood mononuclear cells and plasma in patients with B-NHL before and after chemotherapy.Result:Before chemotherapy,the expression level of APRIL mRNA and protein in patients with Ⅲ and Ⅳ stage of B-NHL were significantly higher than that of Ⅰ/Ⅱ stage(P0.05,respectively),but there were no significant difference between Ⅲ stage and Ⅳ stage(P0.05).After chemotherapy,the expression level of patients with Ⅲ stage of B-NHL were not significantly different from those of Ⅳ and Ⅰ/Ⅱ stage(P0.05,respectively),but there were significant difference between Ⅰ/Ⅱ stage and Ⅳ stage(P0.05).In the treated-patients with B-NHL,the expression levels of APRIL mRNA and protein in PD group were also significantly higher than those of CR and PR(P0.05,respectively),but no significantly difference between CR,PR,and NC groups(P0.05).Conclusion:APRIL may be intimately relative to stage,burden and efficacy.It could be established as a target molecule to treat B-NHL efficiently.
