摘要
目的研究鼻内镜下泪前隐窝进路与唇龈沟进路治疗上颌窦内翻性乳头状瘤的差异,探讨最佳的手术入路。方法回顾性分析鼻内镜下两种手术进路治疗47例上颌窦内翻性乳头状瘤患者的临床资料,统计分析两种手术中出血量、手术时间、术后疼痛时间、住院时间、术后并发症及复发情况的差异。结果泪前隐窝进路在术后疼痛时间、住院时间及术后并发症方面明显优于唇龈沟进路,在术中出血量、手术时间及术后复发率方面两种手术进路无明显差异。结论鼻内镜下泪前隐窝进路治疗上颌窦内翻性乳头状瘤较鼻内镜下唇龈沟进路具有损伤更小、术后反应更轻、住院时间更短、恢复更快、更易于患者接受的优点,值得临床推广应用。
Objective To investigate the difference between endoscopic anterior lacrimal crypt approach and labiogingival groove approach in the treatment of maxillary sinus inverted papilloma for exploring the best operation approach. Methods The clinical data of two approaches in nasal endoscopic treatment of 47 cases of maxillary sinus inverted papilloma were performed the retrospective statistical analysis on the differences in intraoperative blood loss, operation time, postoperative pain, hospital hospital stay, postoperative complications and recurrence between the two approaches of operation. Results Postoperative pain time, hospital stay and postoperative complications in the anterior lacrimal crypt approach were significantly superior to those in the labiogingival groove approach. But the intraoperative blood loss and the recurrence rate had no obvious differences between the two approaches. Conclusion Adopting endoscopic anterior lacrimal crypt approach for treating maxillary sinus inverted pa pilloma has the advantages of smaller lesion, slighter postoperative reactions, shorter hospital stay ,faster recovery and more easily acceptable by patients,which is worthy of being clinically promoted.
