摘要
目的 探讨超声检查对肾上腺无功能肿瘤的诊断价值。方法 对照分析 10 6例肾上腺无功能肿瘤的超声检查和手术病理结果。结果 10 6例患者中 ,超声诊断真阳性 10 3例 ,真阴性、假阳性、假阴性各 1例 ,敏感性、特异性、准确性分别为 99.0 %、5 0 .0 %和 98.1%。肾上腺无功能肿瘤呈逐年增多趋势 ,被超声首次检出者为 77.9%。肿瘤从小到大依次为腺瘤、嗜铬细胞瘤、神经鞘瘤、髓样脂肪瘤、神经节细胞瘤、腺癌。各种肿瘤的大小有极显著性差异。髓样脂肪瘤以高回声为特征 ,易与其他肿瘤鉴别 ,其余肿瘤声像图交叉现象严重。较大的嗜铬细胞瘤、神经鞘瘤常有液化囊变表现 ,腺癌常为不均匀回声。结论 超声检查是肾上腺无功能肿瘤定位诊断首选的可靠的方法 ,并对鉴别肿瘤类型有一定帮助。
Objective To evaluate the localization of adrenal functionless tumor by ultrasonography(US).Methods One hundred and six patients with adrenal functionless tumor were examined by US and subsequently proved by surgery and pathology. Results In 106 cases, true positive was 103, true negative, false positive and false negative were 1 each. The sensitivity, specificity and accuracy was 99.0 %, 50.0 % and 98.1 %, respectively. The adrenal functionless tumor tended increasing year by year. Tumors detected at very first by US were 77.9 %. Tumor size distribution was gradually increasing as following: adenoma, pheochromocytoma(PHEO), neurilemmoma, myelolipoma, gangliocytoma and adenocarcinoma,in which there is significant difference.The myelolipoma presented charactersitically hyperecho feature to be easily distinguished from other tumors. The echo features in the rest tumors wese crossed seriously. The larger PHEO and neurilemmoma often showed up liquid area. Adenocarcinoma often had chaotic echo. Conclusions US is a reliable examination of the first choice for localizing diagnosis of adrenal functionless tumor and also helpful in differentiating histopathologically.
