摘要
目的检测乙型肝炎患者的乙型肝炎病毒表面大蛋白(HBV-LP)及HBV-DNA,探讨两者在乙型肝炎诊治中的互补作用。方法对508例乙型肝炎患者进行HBV-LP(ELASA法)及HBV-DNA(FQ-PCR法)检测和相关分析。结果 508例乙型肝炎患者中"大三阳"组、"小三阳"组、e抗原转阴组和e抗原及e抗体双阴组HBV-LP的阳性率分别为94.7%、73.7%、76.0%和81.5%,HBV-DNA阳性率分别为85.7%、42.3%、50.0%和55.6%,差异有统计学意义(P<0.05或P<0.01);HBV-LP与HBV-DNA检测间有明显的正相关关系(r=0.8420);28例发生YMDD突变标本中HBV-LP阳性率64.3%,10例阴性者HBV-DNA检测全部阳性;14例HBV-DNA检测阴性者HBV-LP检测全部阳性。结论 HBV-LP检测与HBV-DNA检测对乙型肝炎诊治及预后评估有互补作用。
OBJECTIVE To determinate hepatitis B large surface protein （HBV-LP）and HBV-DNA in the patients with hepatitis B and explore the complementary role of the two in diagnosis and treatment of hepatitis B. METHODS The detection of HBV-LP （by ELISA） and HBV-DNA （by FQ-PCR） was performed for 508 patients with hepatitis B. RESULTS Of 508 patients with hepatitis B, the positive rates of HBV-LP of the group DS, XS group, EG group, and SY group were 94. 7%, 73. 7%,76.0% and 81. 5%, respectively, the positive rates of HBV-DNA were 85.7%,42.3 %,50.0 %, and 55.6%, respectively, the differences were statistically significant （P〈0.05 or P〈0.01） ; it showed a significant positive correlation between the detection of HBV-LP and HBV- DNA （r=0. 8420） ; of 28 cases of specimens with YMDD mutations, the positive rate of HBV-LP was 64.3%, all the 10 negative cases were with positive HBV-DNA; and all 14 cases with negative HBV-DNA were with positive HBV-LP. CONCLUSION There is the complementary role of HBV-LP and HBV-DNA detection in the diagnosis and treatment and the prognosis of hepatitis B.
