摘要
目的观察盐酸法舒地尔对冠心病心力衰竭的临床疗效。方法将93例冠心病心力衰竭患者随机分成两组,对照组46例给予常规治疗,治疗组47例在常规治疗的基础上加用盐酸法舒地尔。两组在治疗前及治疗后3个月末进行彩色多普勒超声心动图检查,获得左心室收缩末容量指数和舒张末容量指数及左心室射血分数、左心室体质量指数,治疗过程中观察并记录患者症状、体征、心率、血压及不良反应,治疗前后常规测定血脂、肾功能、肝功能、尿酸、电解质、血糖等各项生化指标的变化。结果盐酸法舒地尔治疗冠心病心力衰竭临床效果明显,临床症状显著改善,总有效率为89.3%,治疗后左心室收缩末容量和舒张末容量明显降低,左心室射血分数明显提高,与治疗前和对照组比较差异有统计学意义(P<0.05),各项生化指标在治疗前后差异无统计学意义(P>0.05),未发现明显不良反应。结论盐酸法舒地尔可有效抑制心室重构,缓解心力衰竭症状。
Objective To evaluate the clinical effects of fasudil hydrochloride on patients with heart failure (HF) secondary to coronary heart disease (CHD). Methods Ninety-three patients with HF secondary to CHD were randomly divided into two groups: control group (46 cases) and treatment group (47 cases). Patients in control group received conventional therapies, and patients in treatment group received fasudil hydrochloride besides conventional therapies. The detection indicators of end systolic volume index, end diastolic volume index, ejection fraction and quality index number of left ventricular of these patients were measured by color Doppler eehoeardiogram on the day and after three months of beginning to accept treatment. The symptoms, physical signs, heart rates, blood pressures and adverse reactions of these patients were observed and recorded in therapeutic process. The biochemical indicators of blood lipid, renal function, liver function, uric acid, electrolytic blood sugar were routinely detected before and after treatment and analyzed the changes. Results Fasudil hy- drochloride had markedly clinical effect for patients with heart failure secondary to coronary heart disease. The clinical symptoms were improved. The effective rate was 89.3%. End systolic volume end diastolic volume of left ventricular dropped and ejection fraction of left ventricular increased after fasudil hydrochloride treatment and were significant differ ence compared to these in pretreatment and control group (P 〈 0.05). There was no significant difference in biochemical in dicators (P 〉 0.05). No adverse reactions occurred. Conclusion Fasudil hydrochloride can effectively prevent left ventricular remod eling and relieve the symptoms of heart failure secondary to coronary heart disease.
