摘要 目的观察手指脱套伤不同治疗方法的临床效果。方法我科2008年7月—2012年1月共收治手指脱套伤44例59指,其中单指脱套伤35例,多指脱套伤9例。分别采用腹部皮管修复术(第1组,21指)、瓦合皮瓣修复术(第2组,20指)、甲瓣或甲瓣和指固有动脉背侧支皮瓣联合修复术(第3组,18指)3种方法进行手术治疗,术后均采用局部烤灯照射及抗菌药物治疗5~7 d。对3组疗效进行观察。结果第1组优5指,良6指,可10指,优良率为52.4%;第2组优9指,良8指,可3指,优良率为85.0%;第3组优12指,良4指,可2指,优良率为88.9%。结论瓦合皮瓣修复术、甲瓣或甲瓣和指固有动脉背侧支皮瓣联合修复术治疗手指脱套伤效果满意。 Objective To observe the clinical effects of different treatment methods on the fingers degloving injury. Methods Among 59 fingers of 44 patients' with finger degloving injury during July 2008 and January 2012 treated in our hospital, 35 fingers were single-finger degloving injury and 9 hyperdactylism degloving injury. Three kinds of treatments were appplied:abdominal skin tube repairing (group 1,21 fingers) , over and out skin flap repairing (group 2, 20 fingers) , pedal nail flap or pedal nail flap combined with free flap repairing (group 3, 18 fingers). Local heat lamp irra- diation and antibiotic therapy were given for 5-7 days after operation. Comparative observation on clinical effect was taken in three groups. Results In group one clinical efficacy were excellent in 5 fingers, good 6 and common 10, and the excellence rate was 52.4% ; in group two, excellent in 9 fingers, good 8 and common 3, the excellence rate 85.0% ; in group three, excellent in 12 fingers, good 4 and common 2, the excellence rate 88.9%. Conclusion Satisfactory effect will be achieved in treating the finger degloving injury with the over and out skin flap repairing and pedal nail flap or pedal nail flap combined with free flap repairing.
