摘要
目的:简述中成药在综合医疗机构的使用现状,分析存在的问题,为促进中成药在综合医院的合理使用提供参考。方法:通过查阅相关文献及国家药品不良反应监测中心公布的《药品不良反应信息通报》,结合我院门诊中成药的使用情况,抽查2011年12月～2012年2月各月第1周门诊处方共计6619张,对中成药在综合医疗机构使用中存在的问题及其原因进行简要分析;并就应对措施及建议进行了简单梳理。结果:通过处方抽查及文献复习,发现综合医疗机构的中成药处方多为西医开具,中成药的用量呈逐年上升趋势,但在使用中存在一些问题,主要表现在用药不规范、不合理联合用药。结论:导致综合医疗机构中成药不合理用药的原因主要是临床西医师缺乏中医药理论的指导,中成药药品说明书不够完善,临床医师对中成药毒副作用重视不够等。为促进中成药的合理使用,相关综合医疗机构应加强临床医师中医药基础理论知识的培训,加大对《中成药临床应用指导原则》的宣传和执行力度,同时建立中成药处方点评制度,临床医师还可积极参与中成药上市后的临床再评价工作。
Objective： Briefly recount the status quo of proprietary Chinese medicines usage in the integrated medical institutions,to analyze the problems and reasons,and provide a reference for promoting rational proprietary Chinese medicines use in integrated medical institutions.Methods： By means of looking through relevant literature and the National Adverse Drug Reaction Monitoring Center released ＂Adverse Drug Reaction Information Bulletin,＂ combine with the use of proprietary Chinese medicines in our hospital,6619 prescriptions from Dec.of 2011 to February 2012 were collected and analyzed,in which the prescriptions were collected in the first week of each month.Then the response measures and recommendations were also simply combed.Result： By prescription checks and review of the literature,we found that in the integrated medical institutions most proprietary Chinese medicines prescription were written by physicians,and the amount of proprietary Chinese medicines showed an increasing trend,but there are some problems in use,mainly in the medication is not standardized,unreasonable combination administration.Conclusion： The reasons leading to irrational drug use of proprietary Chinese medicines in integrated medical institutions were that many of the physicians lack the guidance of the theory of traditional Chinese medicine,proprietary Chinese medicines instructions were not perfect,the clinicians′ inadequate attention to the side effects of proprietary Chinese medicines.To promote the rational use of proprietary Chinese medicines,integrated medical institutions should strengthen the training of basic theoretical knowledge of Chinese medicine clinicians,increased publicity and enforcement of proprietary Chinese medicine clinical application Guiding Principles,at the same time established a proprietary Chinese medicine prescription review system,and clinicians could actively participate in clinical re-evaluation of proprietary Chinese medicines.
