肺硬化性血管瘤^(18)F-FDG PET/CT表现及临床意义(3例报道并文献复习) 被引量:2

摘要 目的:探讨硬化性血管瘤的PET/CT表现及显像机制,提高对PSH的影像学认识。方法:回顾性分析从2005年2月至2011年9月于我院行18F-FDG PET/CT检查并经病理确诊的3例硬化性血管瘤患者的临床及18F-FDG PET/CT资料,结合近15年国内外文献报道的行18F-FDG PET/CT检查的13例(国外12例,国内1例)硬化性血管瘤患者共18个病灶资料,进行相关文献复习。结果:2例为单发病灶,1例为双肺多发病灶(多发病灶仅将最大者纳入分析)。2例位于肺下叶,1例位于左肺上叶;大小约1.8 cm×2.7 cm×1.7 cm~2.8 cm×2.4 cm×3.0 cm;3例均表现为边界清晰的类圆形结节影,1例边缘浅分叶,2例可见短毛刺;2例密度均匀,1例可见小钙化;糖代谢轻、中度增高,SUVmax 1.7~5.4。文献复习示硬化性血管瘤同机CT多表现为单发的边界光滑清晰的圆形或类圆形结节/肿块,位于双肺下叶者多见,平均直径(3.0±1.1)cm(1.7~5.5 cm),密度较均匀,偶见钙化,少见分叶、毛刺,周围可见肺气肿,偶见多发或淋巴结转移。18F-FDG PET表现为轻中度放射性浓聚,平均最大SUV 3.0±1.2(1.6~5.4)。大小与FDG摄取高低无相关性。结论:18F-FDG PET显像示硬化性血管瘤糖代谢多呈轻中度增高,摄取程度可能与细胞类型有关。不典型的硬化性血管瘤及多发硬化性血管瘤易误诊为恶性肿瘤,应谨慎鉴别。 Objective:The pulmonary sclerosing hemangioma(PSH) is a relatively rare benign tumor of the lung. Few studies have been reported addressing its PET/CT characteristics. The aim of our study was to investigate the PET/CT findings of PSH and discuss its relevent factors which can imqeunce the uptake. Meth- ods: 3 cases with PSH confirmed by pathology of our PET/CT center were presented, and relevant literatures were retrospectively reviewed. The relation between tumor size and its SUVmax were analyzed. Results:One case of the three showed diffused lesions of both lungs, the other two demonstrated single lesion. For multiple fo- ci, We just brought the largest one into analysis, for the others were too small to be detected by PET/CT. All 3 lesions were detected byTM F-FDG PET/CT. Two located in the lower lobe ,while the other one in the left upper lobe. The size of the lesions ranged from 1.8 cm×2.7 cm×1.7 cm-2.8 cm×2.4 cm×3.0 cm. All the three tumors showed round nodules with clear borders. One showed lobule and two lesions had burrs. One lesions re- vealed calcification, while the other two owned even density. The uptake of the lesions were low to mediate, with SUVmax ranging from 1.7 to 5.4. Totally 18 lesions of 16 cases including our 3 cases were analyzed. The char- acteristic CT scan findings showed a solitary round or oval well-de findings nodule, predominantly located in the lower lobes of both lungs. The tumor always showed even density ,with calci findings tions sometimes presented. Emphysema near the tumor occasionally could be seen. The mean diameter of the 18 tumors was ( 3.0 ± 1.1 ) cm ( range : 1.7 - 5.5 cm) with mean SUVmax 3.0± 1.2 ( range : 1.6 - 5.4 ). The maximal standard uptake valueof FDG uptake had no correlation with the tumor size ( r = 0. 257, P 〉 0. 05 ). Conclusion: Typical PSH showed well defined round nodule with PET/CT showing low to mediate uptake. The mechanism of FDG uptake maybe contributes to the cell types. The possibility of false-positive result caused by PSH should be considered while interpreting FDG PET images of solitary pulmonary nodules.
