葛根对盆底胶原蛋白代谢作用的研究 被引量:2

摘要 目的研究葛根对盆底功能障碍性疾病患者盆底阴道壁粘膜及韧带组织中I型和Ⅲ型胶原蛋白代谢影响,以探讨其临床应用价值。方法选择60例绝经后的盆底功能障碍性疾病患者并行阴式全子宫切除手术,30例术前予葛根治疗30天的患者为用药组,其余30例未使用任何外源性雌激素的患者为对照组。术中取阴道壁粘膜组织及宫骶韧带组织。再采用免疫组化二步法和图像分析检测I型和Ⅲ型胶原蛋白的含量。结果免疫组化及图像半定量分析结果显示,用药组阴道壁粘膜组织I型胶原阳性细胞或强阳性细胞的表达率均显著高于对照组(t值分别为13.09、12.14,均P〈0.01);用药组宫骶韧带组织I型胶原阳性细胞或强阳性细胞的表达率均显著高于对照组(t值分别为12.23、8.88,均P〈0.01);用药组阴道壁粘膜组织Ⅲ型胶原阳性细胞或强阳性细胞的表达率均显著高于对照组(t值分别为8.74、6.46,均P〈0.01);用药组宫骶韧带组织Ⅲ型胶原阳性细胞或强阳性细胞的表达率均显著高于对照组(t值分别为6.68、5.92,均P〈0.01)。结论葛根通过增加盆底胶原蛋白的含量,能部分改善盆底功能障碍性疾病的症状,可以用于年老不能耐受手术的盆底功能障碍性疾病的术前准备以及绝经后轻度盆底功能障碍性疾病的患者。 Objective To investigate the effect of pueraria on metabolism of collagen type I and collagen type Ⅲ in vaginal wall tissues and ligament tissue in patients with pelvic floor dysfunction (PFD), so as to discuss its clinical value. Methods Sixty post-menopause women with PFD and receiving vaginal hysterectomy were enrolled in the study, including 30 patients using pueraria for 30 days before receiving vaginal hysterectomy ( medication group) and the other 30 patients without taking any containing estrogen medicine ( control group). Vaginal wall tissues and uterosacral ligament tissues were collected during surgery, and then the histomorphologic structure of them were observed under microscopy and the contents of collagen type I and collagen type Ⅲ were detected by immunohistochemistry. Results Immunohistochemistry and histomorphologic analysis revealed that the expression rates of positive or strong-positive collagen type I cells in vaginal wall tissues in medication group were significantly higher than those in control group (t value was 13.09 and 12.14, respectively, both P 〈0.01 ), so were in uterosacral ligament tissues (t value was 12.23 and 8.88, respectively, both P 〈0.01 ). The expression rates of positive or strong-positive collagen type III cells in vaginal wall tissues in medication group were significantly higher than those in control group ( t value was 8.74 and 6.46, respectively, both P 〈 0.01 ), so were in uterosacral ligament tissues ( t value was 6.68 and 5.92, respectively, both P 〈 0.01 ). Conclusion Pueraria can partially improve PFD symptoms by increasing the content of collagen in pelvic supportive tissue. Therefore, pueraria can be taken as a kind of promising agent used for the aged PFD patients who cannot undergo surgery or for preoperative preparation, and as the conservative treatment drug for menopausal patients with mild PFD but not influencing life quality.
