鼻唇沟双叶皮瓣修复鼻下部小面积皮肤缺损 被引量:4

A novel bilobed nasolabial flap for the repair of distal nasal defect

摘要 目的探讨鼻唇沟双叶皮瓣修复鼻下部小面积皮肤缺损的方法和疗效。方法结合zi-telli和邢新的设计原则,在鼻部缺损外侧鼻唇沟区域设计蒂部在上的双叶皮瓣,第一叶皮瓣面积略大于缺损面积,第二叶皮瓣与第一叶皮瓣面积相当或略小,双叶皮瓣共旋转90。-100。观察术中切口张力、术后即刻鼻子外形变化;术后随访,观察远期修复效果。结果本组14例患者,缺损均位于鼻下部接近中线处,呈圆形或椭圆形。缺损面积最小者0.8cm×1.0cm,最大者1.2cm×1.8cm。术中缝合切口无明显张力,术后即刻鼻外形无明显变化。随访10—18个月,术区无明显畸形,皮瓣色泽、质地匹配良好,瘢痕纤细、隐蔽,效果满意。结论鼻唇沟双叶皮瓣修复鼻下部小面积皮肤缺损,效果良好,值得推广。 Objective To evaluate the design principle, clinical result and significance of a novel bilobed nasolabial flap for the repair of distal nasal defect. Methods Based on the designs of Zitelli and Xing Xin, a su- periorly based bilobed flap in the nasolabial region was planned. The axis of flap rotation is 90~ to 100% The length of the primary lobe was slightly longer than the distance of the distal defect edge to the flap's pivot point, and the width of the primary lobe was equal to the width of the defect. The length of the second lobe was a few larger than that of the primary lobe and the width of the second lobe was similar to or slightly smaller than that of the primary lobe. Outcome measures included ability to close the defect with minimal tension, cosmetic appear- ance, and complications. Results The bilobed nasolabial flaps were performed in 14 cases with distal nasal de- fects. The size of defects ranged from 0.8 cm x 1.0 cm to 1.2 cm x 1.8 cm. The incisions were sutured with mini- mal tension. All flaps survived completely with ,good colour and texture match. There were not dissymmetry de- formity and retard deformity of nasal ala. All scars were almost invisible, and all cases were satisfied with their outcome. Conclusion This kind of bilobed flap is novel and can be a useful alternative procedure to close small distal nasal defects. Its aesthetic outcome is good, and worth spreading in clinic.
