摘要
目的探讨学龄期先天性甲状腺功能减低症(CH)儿童抑郁及社会支持状况。方法选取南京市经新生儿筛查确诊的永久性CH学龄期儿童45例(CH组)及性别、年龄与之相匹配的健康儿童45例(健康对照组),2组儿童均接受Zung氏抑郁自评量表(SDS)评价其抑郁情况,采用肖永源社会支持评定量表进行问卷调查,分析其社会支持状况。结果 CH组儿童抑郁发生率明显高于健康对照组(31.11%vs 6.67%),CH组儿童SDS正常标准分明显高于健康对照组[(47.56±10.13)分vs(32.75±7.67)分],差异有统计学意义(P<0.05)。CH组儿童的客观支持分、主观支持分、对支持的利用度评分和社会支持总分均明显低于健康对照组(Pa<0.05)。CH儿童的抑郁状况与社会支持总分、主观支持以及对支持的利用度均存在负相关(Pa<0.05);但与客观支持无相关性(P>0.05)。结论学龄期CH儿童抑郁发生率较高,SDS正常标准分升高,得到的社会支持度偏低;应给予CH儿童针对性的主观支持和心理干预,以减少CH儿童抑郁症状的发生和发展。
Objective To investigate the depression and social support in school children with congenital hypothyroidism (CH). Methods Forty -five school children with permanent CH (CH group) diagnosed by neonatal screening were studied and compared to 45 matched healthy children( healthy control group) born at the same time. The school children in the 2 groups were investigated by question- naires,their depression status were evaluated by Zung's self- rating depression scale (SDS), their social support were analyzed by XIAO Yong - yuan's social support rating scale. Results The incidence rate of depression in CH group was significantly higher than that in healthy control group (31.11% vs 6.67% ), the SDS standard score of children in C H group was significantly higher than that in healthy control group [ (47.56±10.13 ) vs( 32.75 ± 7.67 )], the difference was significant( P 〈 0.05 ). The scores of subjective support, objective support and availability of social support, the total scores of children in CH group were significantly lower than those in healthy control group( Pa 〈 0.05 ). The CH children's depression status and subjective support and availability of social support,the total scores had negative correlations( Pa 〈 0. 05 ), but was not related with the objective support ( P 〉 0.05 ). Conclusions The school children with CH have higher incidence rate of depression,higher SDS standard score and lower social support degree. They should be given to the psychological intervention and subjective support, so as to reduce the occurrence and development of depressive symptoms.
