摘要
目的应用甲泼尼龙琥珀酸钠和不同剂量乌司他丁干预脓毒症大鼠,观察两种药物对脓毒症大鼠免疫功能的影响。方法40只健康成年SD大鼠,随机分为5组,分别为:空白对照组、脓毒症组、甲泼尼龙琥珀酸钠组、常规剂量乌司他丁组、大剂量乌司他丁组,每组各8只(n=8)。静脉注射大肠杆菌内毒素制作脓毒症大鼠模型。造模后分别给予上述药物干预的同时,皮下注射40mL/kg生理盐水抗休克治疗,观察各组试验动物平均动脉压的变化。给药后4h取血,测定免疫球蛋白G(IgG)和淋巴细胞CD3、CD4、CD8阳性的百分率及CD4/CD8比值。结果脓毒症组与空白对照组相比,IgG、CD3、CD4、CD8皆明显下降,两组之间的差异具有统计学意义(P〈0.05);大剂量乌司他丁组与脓毒症组、甲泼尼龙琥珀酸钠组、常规剂量乌司他丁组相比IgG、CD3、CIM、CD8皆升高,差异均具有统计学意义(P〈0.05);其余各组之间CD4/CD8比值及在所有组与组之间的变化均无统计学意义(P〉0.05)。结论脓毒症大鼠较正常大鼠免疫功能显著降低;常规剂量乌司他丁、甲泼尼龙琥珀酸钠对脓毒症大鼠免疫功能改善不明显;大剂量乌司他丁能改善脓毒症大鼠体液及细胞免疫功能。乌司他丁对脓毒症大鼠免疫功能的改善可能呈剂量依赖型;大剂量乌司他丁对于脓毒症大鼠免疫功能的改善明显优于甲泼尼龙琥珀酸钠。
Objective The aim of this study is to investigate the effctions of methylprednisolone sodium succinate and different doses of UTI on septic rats, to provide a probable methods for treating septic patients. Methods 40 SD rats, were randomly divided into 5 groups: control group, sepsis group, methylprednisolone sodium succinate group, conventional dose ulinastatin group, high dose ulinastatin group, 8 rats in each group. Septic model of rat is made by injecting E. coli endotoxin in veins, then the drugs were given separately, at last a dose of saline (40 mL/kg) was subcutaneous injected simultaneously for fluid resuscitation; all the animals were observede intensively the changes of mean arterial blood pressure, after 4 hours from the drug administration, blood samples were taken to be centrifugated, and the supernatant was used for quantifying imnmnoglobulin (IgG); another blood sample were separated by lymphocyte liquid extraction lymphocytes for flow cytometry. The percentage of lymphocyte- CD3, CD4, CD8 and the ratio of CD4/CD8 were determined. Results IgG, CD3, CD4 and CD8 were significantly decreased in sepsis group than in control group ( P 〈 0.05 ) ; IgG, CD3, CD4 and CD8 were rising in high dose of ulinastatin group than in sepsis group, conventional - dose ulinastatin group, methylprednisolone group ( P 〈 0.05 ). There are no statistical changes of CD4/CD8 ratio in all groups(P 〉 0.05 ). Conclusion compared with normal rats, the immune function in septic rats were significantly depressed; both Conventional - dose of ulinastatin, and methylprednisolone sodium succinate have no effects on immune function in septic rats. Compared with conventional -dose ulinastatin, high -dose of ulinastatin may improve the humoral and cellular immune function in septic rats. The data suggest ulinastatin may improve immune function in septic rats in a dose dependent manner; high -dose of ulinastatin for improving the immune function of septic rats may be superior to methylprednisolone sodium succinate.
