帕金森病患者血清TNF-α、sTNFR1、sTNFR2水平的检测及临床意义 被引量:10

摘要 目的检测帕金森病(PD)患者血清TNF-α、sTNFR1、sTNFR2水平,分析彼此的相关性,探讨其变化的原因及相关因素。方法采用生物素双抗体夹心酶联免疫吸附法,对40例PD患者及性别、年龄匹配的30例正常对照组血清TNF-α、sTNFR1、sTNFR2水平进行检测。采用PD统一评分量表第3部分(UPDRS III)和改良Hoehn-Yahr分级对PD患者病情进行评估。并按照治疗状况对PD患者进行分组。结果 PD组血清sTNFR1水平显著高于对照组(P<0.01);PD组TNF-α和sTNFR1水平低度正相关(rs=0.40,P<0.05),sTNFR1和sTNFR2水平显著正相关(rs=0.55,P<0.01),对照组血清sTNFR1和sTNFR2水平中度正相关(rs=0.42,P<0.05);对PD组血清TNF-α、sTN-FR1、sTNFR2水平与年龄、病程及病情进行相关性分析,结果表明TNF-α水平与年龄、病程、UPDRS评分及Hoehn-Yahr分级均无相关性,sTNFR1和sTNFR2水平与病程、UPDRS评分及Hoehn-Yahr分级均无相关性,但sTNFR1水平与年龄显著正相关(rs=0.497,P<0.01),sTNFR2水平与年龄低度正相关(rs=0.353,P<0.05);PD组按治疗情况进行分组,各组间TNF-α、sTNFR1、sTNFR2水平均无显著性差异(均P>0.05)。结论 PD患者血清sTNFR1水平显著增高,且与年龄正相关,提示TNF-α介导的细胞内信号级联反应参与了PD的免疫炎性病理过程,老化是主要危险因素。同时检测两个sTNFRs水平对TNF-α的整体水平评估是必要的,它们是比TNF-α更可靠的标记物。 Objective To detect the serum concentration of TNF-α,sTNFR1 and sTNFR2 in Parkinson's disease patients,to analyze the correlations between the levels of TNF-α,sTNFR1 and sTNFR2 in Parkinson's disease(PD) patients and to investigate the causes and correlative factors of serum TNF-α,sTNFR1 and sTNFR2 level change in PD.Methods There serum TNF-α,sTNFR1 and sTNFR2 levels of 40 patients with PD and 30 healthy subjects with age and sex-matched were measured by Enzyme-Linked Immunolsorbent Assay(ELISA).The United Parkinson's Disease Rate Scale Part III Score(UPDRS III)and Modified Hoehn and Yahr Staging Scale(HY) were evaluated in all studied PD patients.Results sTNFR1 level of PD group is significantly higher than the healthy subjects group(P〈0.01);There was a low positive correlation between the levels of TNF-α and sTNFR1 in PD group.(rs=0.40,P〈0.05),there was a significant positive correlation between the levels of sTNFR1 and sTNFR2 in PD group(rs=0.55,P〈0.01),there was a modetate positive correlation between the levels of sTNFR1 and sTNFR2 in healthy subjects.There was no significant correlation TNF-α and age,duration,UPDRS III score,Hoehn-Yahr Staging Scale in PD group,there was no significant correlation between sTNFR1,sTNFR2 and duration,UPDRS III score,Hoehn-Yahr Staging Scale in PD group,there was a significant positive correlation between sTNFR1 and age in PD group(rs=0.497,P〈0.01),there was a low positive correlation between sTNFR2 and age in PD group(rs=0.353,P〈0.05).The PD group was divided into different the state of therapies of illness,there is no significant difference.Conclusion Serum sTNFR1 levels were significantly higher and positively correlated with age in PD patients,suggesting that TNF-α mediated intracellular signaling cascades involved in the PD inflammatory pathological process,aging is the major risk factors.Simultaneously measurement of circulating levels of the two sTNFRs is useful to determine the overall production of TNF-α,which may be considered more reliable markers of inflammatory activity than TNF-α.
