摘要 目的探讨CT影像和尸头解剖在单鼻孔经蝶窦入路切除垂体瘤中蝶窦的神经血管显示及其解剖位置定位中的应用,为该术式提供重要的解剖依据,避免损伤周围组织。方法收集18具新鲜尸头并通过显微镜经鼻腔-蝶窦入路研究蝶窦解剖结构,同时对35例志愿者进行蝶窦CT平扫+增强扫描并进行CTVE三维重建,两方法均测量蝶窦到周围组织的距离,鞍底血管、神经的显示情况。结果 (1)两种方法均能较好的测量蝶窦到周围组织的距离,除蝶窦口到蝶筛隐窝顶外(P<0.05),其余均没有差异;(2)两种方法在蝶窦表面解剖结构的显示率上没有差异,但CT影像在颈内动脉视神经隐窝的显示率上高于尸头解剖(P<0.05),但其不能显示斜坡松质骨。结论 CT影像在单鼻孔经蝶窦入路切除垂体瘤中蝶窦的神经血管显示及其解剖定位中应用效果较好,可为术前单鼻孔经蝶入路方案的制定提供依据。 Objective To explore the application of CT images and the anatomy of cadaveric heads to investigate the anatomical location and display of sinus nerves and blood in sphenoid sinus in the removal of pituitary tumor by endonasal transsphenoidal surgery,and serve as a clinical reference for the surgery to avoid damaging nearby organs.Methods 18 fresh cadaveric heads were collected to investigate the anatomical structure by microscope,and 35 volunteers were given sphenoid sinus CT scan and enhanced scan as well as three-dimensional reconstruction.The distance from sphenoid sinus to the surrounding tissue,and the display of blood vessels and nerves were both measured in two methods.Results(1)Both Methods can measure the distance from sphenoid sinus to the surrounding tissue.There were no difference among the rest distances,except the distance from ostia of sphenoid sinus to the roof of the sphenothmoid concavity(P0.05).(2)There were no significant difference between the two Methods on the display of the superficial anatomic structures of sphenoid sinus but the display rate of opticocarotid recesses of CT was superior to that of anatomy.Conclusion CT image is very effective in measuring the anatomy location and displaying the sphenoid sinus nerves and blood vessels,worth to be promoted clinically.
