呼出气一氧化氮在慢性咳嗽患者吸入糖皮质激素疗效评价中的价值 被引量:5

Relationship of Exhaled Nitric Oxide and Inhaled Corticosteroid Treatment Response in Patients with Chronic Cough

摘要 目的:探讨检测呼出气一氧化氮(FeNO)在慢性咳嗽吸入糖皮质激素(ICS)治疗疗效评价中的作用。方法:回顾性分析62例慢性咳嗽患者临床资料,依ICS治疗结果分为应答组40例和无应答组22例,对2组患者行FeNO检测以及支气管舒张试验,分析2组一般临床资料及诊断结果差异,并比较不同FeNO水平时ICS治疗应答的敏感性和特异性。结果:2组年龄、性别、体质量指数、FEV1%预测值等差异无统计学意义(P>0.05)。ICS治疗应答组FeNO的平均水平高于无应答组,差异有统计学意义(P<0.05);判断ICS治疗有无应答的FeNO的最佳临界值为34.1ppb。结论:FeNO水平对慢性咳嗽患者的ICS治疗效果有预测作用,可为慢性咳嗽患者选择ICS治疗提供依据。 Objective: To evaluate the effect of fraction of exhaled nitric oxide (FeNO) on inhaled corticosteroid (ICS) therapy in patients with chronic cough. Methods: Clinical records were extracted in this retrospective observational study of 62 patients. Patients were classified into two groups with respect to the response to ICS treatment, group 1 (response to ICS, n=40) and group 2 (nonresponse to ICS, n=22). The exhaled nitric oxide measurement and bronchial dilation test were performed in two groups. The clinical data were analyzed and the sensitivity and specificity of FeNO level and ICS treatment response were detected in two groups. Results: There were no significant differences in age, gender, body mass index (BMI) and FEVI% predicted value between two groups (P 〉 0.05). The FeNO levels were significantly higher in response to ICS group than those of nonresponse to ICS group (P 〈 0.05). A cutoff of 34.1 ppb was found to be the best differentiation of ICS response. Conclusion: The measurement of exhaled nitric oxide accurately predicted the response to ICS therapy in patients with chronic cough. This finding may potentially have an impact on ICS treatment in chronic cough patients.
